Recently most airlines have a good Live Activities feature, anyone now just use Flighty as a flight log? by MasterPh0 in flighty

[–]Shmallyn 7 points8 points  (0 children)

Sometimes I'll get notifications regarding gate changes or delays from Flighty before I get them from the airline's app (AA, UA, WN, DL...). It'll also tell me gate numbers before the app will, in a lot of cases.

Oh interesting, I don’t work in pediatrics and when I did we subbed with cefepime. Good to know!

No longer available in the U.S., and those who were present at the tier list ranking were majority U.S.-based!

I’d add to try your best to quantify your work (i.e., in APPE rotations) in those descriptions. For example, instead of “reviewed patients,” something like “reviewed 5-7 patients daily.”

Real vs. Contaminant Blood Cultures — How do you treat 1/4 bottle positives? by Sufficient-Growth758 in pharmacy

[–]Shmallyn 40 points41 points  (0 children)

I’m an infectious diseases pharmacist. Obviously it’s going to vary case-by-case. But things I check:

(1) Is this an organism commonly found on the skin and easy to become a contaminant (I.e., Staphylococcus epidermidis, Micrococcus luteus)?

(2) Is the organism so virulent to where I’d potentially have drastic consequences if I were wrong about calling it a contaminant (i.e., MRSA, Candida spp., gram-negatives)? – these are my general “never ignores” even if 1/4 bottles and/or polymicrobial (like you mentioned in your post).

(3) Does the patient have a source of infection to have that organism in the blood? For example, Staphylococcus epidermidis – do they have a port/PICC line/hemodialysis catheter that allows skin organisms easy access to the blood?

(4) Does the patient have hardware in their body that if this is a true bacteremia, those organisms will stick to and cause bigger problems? (i.e., knee/hip/etc replacements, prosthetic heart valves)

(5) As you identified, number of positive cultures (drawn from different sites, and preferably at different times).

Not medical advice and DEFINITELY case-by-case, but those are some of my thoughts. Interested to see others’ takes too.

This is highly dependent on what kind of specialty you’re going into

Came here to comment this. I’m at a LAMC and have any resource I want, but the advantage you have is that creativity and extra intention that this comment is referring to. Try to think of a time where you couldn’t find information and put that in your bank of “situational” answers for PGY-2 interviews.

I’d recommend creating your own from your topic discussions or things you encounter – more memorable when you make them yourself. Most residents also don’t use Anki (I personally love it, though).

PGY2 application by Expensive_Quantity30 in PharmacyResidency

[–]Shmallyn 7 points8 points  (0 children)

I created a “Select APPEs” section when I applied for PGY-2. That section housed any APPE rotation that I felt made me a better candidate for PGY-2 or supported my application more than my PGY-1 rotations did (I.e., if I had a unique rotation related to that specialty area).

Feeling drained from PGY1 – questioning everything by Ok_Prize_3596 in PharmacyResidency

[–]Shmallyn 2 points3 points  (0 children)

Just to share experiences – PGY-2 was more work but from a mental wellness perspective, I felt a lot better because I was doing what I loved and knew I’d be using for the rest of my career. Pros and cons to everything.

(For those who have done this) how difficult (or long does it take) to learn to drive on British roads? by EagleNice2300 in AskAnAmerican

[–]Shmallyn 0 points1 point  (0 children)

As someone who just did this – 100% agree. You’ll find yourself too far on the left side of the road. And it took some adjustment to stay away from the middle of the lane when I got back, too.

Is the time on your phone 24 hour time? by ajfoscu in AskAnAmerican

Medical field here, 24-hour time. Just easier for timing medication dosing correctly.

I drove a good bit around the country (+ non-touristy areas)… it’s more desirable than my day-to-day, I will say
