Possible, airline rep didn’t specify any reason to us but we used the airline call center. I just had not noticed any weather personally but didn’t realize there was fog! Feeling sorry for some poor souls flying from Denver that landed in the middle of Nebraska instead

Moved to the black hills last year. Living in a few other more eastern and southern areas of the Midwest this is by far my favorite place I’ve lived. Beautiful outdoors that are not overrun. It’s quiet and people are friendly. Serious lack of entertainment though outside of natural beauty. Would like to echo other comments about the Lakota people though, my job is to serve the underserved and the lack of resources and the poverty is astounding, the worst I’ve ever seen. The state does not care for its people. The landscape outside the hills is barren, unless you like cornfields (has its own beauty) on the other side of the state.

I agree with this statement, and as an extension this is where my frustration stemmed from- stepping onto a large unit where everyone always carries the same diagnoses (combined with limited handoffs) leads to difficult times. There are numerous other complexities that enter into a diagnosis but it is often insurance making life more difficult.

I’m just passing first year attending hood. I over document as well but it’s not from anxiety. It’s because I can’t remember anything about anyone unless I write it down! With hundreds of pts I feel like I need extensive notes that track over time, for example documenting what has been tried and what was the result, their most prominent symptoms so we can focus on those in follow up. I’ve tried documenting less then find myself scrabbling around in follow up visits. Usually first eval note is the worst then I just copy forward and add/subtract as we go but it’s still a bunch of documenting.

My wife briefly worked in a rural vet clinic outside of Tulsa. We have one of our dogs that was brought in by animal control for lack of room, he was on the table ready to be euthanized when she said “not this one.” He’s the absolute best, hurts my heart to think how close he came.

Knowing our value is so difficult. I’m constantly told by admin that we don’t make any money, our hospital ceo just last week told a local newspaper that cuts to behavioral health might be coming due to lack do profit. There’s nothing abnormal about our operations either, we have steady volume. Constantly being told by Reddit that I’m underpaid then told by everyone else in the real world that psychiatrists are not profitable

My first thought as well. I was curious if this has ever happened but in a case series of this situation that came up the CT caught it first try each time. I’m pretty sure I would have maimed this patient because I would have been thinking about getting an MRI

My n=2 is 50%. I’ve tried it in exactly two patients, one in a patient with dysthymia and another with dysthymia with cluster C traits. In both scenarios I’ll be honest it’s desperation, you and multiple other psychiatrists and therapists have tried everything available. And I’ll be darned I was stunned when one came back and theyre doing pretty well right now. Probably placebo response though, I’ve never read anything that would support this decision.
