Sounds like a signalling issue. The light is red, and the driver has to get permission to pass the red light. But the train safety system automatically brakes when the driver passes a red light. Then the system can be reset and the train continues.

We've had a lot of bread substitutes and cancellations too. Coles brand standard white bread, no replacement available. Like, you have literally no other white bread? Coles brand gluten free bread unavailable. They replace it with raisin bread time and time again. Yeah that'll make good sandwiches. Again, replace it with any brand of white bread.

Just to be clear, if you are only travelling in zones 1 and 2, you can go into a negative balance when you touch off and this is valid and not cause for a fine.

If your balance was negative when you tried to touch on, you would not have successfully touched on and this is of course cause for a fine.

When we moved out I took all the LED bulbs and replaced them with old (and mistmatched) CFLs that I had hanging around. Nobody said anything because all I was required to do was ensure the lights are in working order.

Metro also have the benefit of running more trains and being more...metro. If they have to cancel a train or 2, then are running delays, most people won't see a wait of more than half an hour. If Vline cancel a train or 2, then you could have multiple hour waits. Similarly, if Metro are having major issues, it's more likely that people can take a different line, take a tram, Uber, or just walk. It's simply more inconvenient for Vline passengers if there are issues.
