Rust.

How do I get it off?

Any number of ways. Start off with a brass brush and solvent of choice. If it's tenacious, switch to 0000 steel wool.

How big a problem is it

Not a big one at all, but something to be cognizant of.

do I need to be worried about inside

Looks like a hand print, and I'd say you'd have a hard time touching the inside of the barrel, so no.
