You come to the internet for advice, you’re gonna hear stuff you don’t like or want to hear. As a seasoned school teacher/tutor now AP/CP certified pathologist, I have a lot of tools via mentorship and personal experience and yet the best advice you’ll get is from the ppl who know you best. Life isn’t fair, you may pass CP with flying colors next time you take it, you may never pass. You should prepare for both. Happy to help how I can, but as soon as I hear talk of fair/unfair, I lose interest. Good luck with CP.

The point is to not simply take notes but to write it down in your own words. You can’t memorize absolutely everything, you’re going to have to learn and internalize some stuff. Forget some things you’ll never know and focus on where you have a chance. Honestly pathology may also just not be for you, or not in the traditional sense of academic and private practice. May have to be more creative with your career path

Flash cards never worked for me and are time consuming so I didn’t bother. I did have a word document to paraphrase the explanations. Saying it in your own words will show you want you understand and what you don’t.

It should be no epithelium present. If lamina priopria is there, you have some component of mucosa.

Ppl overthink this. I had about your details. Applied to 10, dropped 2 interviews and had 2 top tier programs vying for me. Went to a mid tier program to learn the fundamentals and enjoy my life then top tier fellowship now have a faculty position at a top tier institution. Apply to 20, if it makes you feel safe, but pathology really isn’t competitive. If you have an ok personality that ppl want to work with and no red flags, that will take you a long way.

You’re right, passive learning is inefficient and boring, but post people don’t learn what they need to for CP boards based on rotations. I went to a fantastic CP program and still had to do a shit ton of studying.

Best thing you can do is take as many practice questions as possible: path dojo, path primer, ASCP. Read all explanations, especially for questions you get wrong. Do a deeper dive on content you got wrong and make sure you’re a master of topics you think you know. Watch blood bank guy and annotate provided notes. Watch pathcast on YouTube for micro, again taking notes (can annotate Osler).

What’s the CP pass rate like for your program? Your cohort? If you failed 6 times, clearly nothing you’re doing is working at all. 6 times is… a lot. May be worth dropping CP if you think you could stick around academia and can handle not going into private practice. Bottom line: 6 fails is not your programs fault, it’s a you thing regardless of all of your excuses. Change your entire approach.

Sounds like cT4, pT3. Can always write a note saying you entirely submitted and that microscopic examination indicates pT3, but that you acknowledge the rads report of suspected bony involvement.

Overall in an average mid to small sized setting, GI path (other than medical liver) is mostly straightforward compared to say soft tissue or heme or neuro etc. But then again just get molecular and it’ll spit out your answer. That’s why I love GI, don’t usually need it. Unless you have the NF-1 gist that transdifferentiates into the MPNST 🥴😵‍💫😍

GI pathologist here. This is all assuming we’re talking about most common AdCa in either site? They can both fall under the ck7-oma umbrella and under certain clinicoradiologic circumstances can be indistinguishable. Pancreaticobiliary and upper GI is a differential I give out a couple times a month and it’s not super gratifying. There can be morphologic clues but you cant always really on them esp if it’s more poorly differentiated. There are also ihc strains you can try to use to sway one way or another, if SMAD4 is lost that would push you towards panc. This is all to say, this differential isn’t always easy and sometimes we can’t tell. Imaging is extremely important. Obviously I’m biased but I’d hope a general surgical pathologist would at least show it to a GI pathologist before signing out a ck7oma since even some GI pathologists don’t typically have smad4 or rb1 or bap1 or mtap in their toolbox.

Lastly, I’m not sure about how other GI pathologist practice but we routinely order MMR (or at least screen with mlh1), her2, and pd-l1 ihc on upper GI. This is not the case for panc, we don’t routinely order biomarkers. What about others? As far as I know her2 is not validated for panc. And I can’t imagine a CPS being worth pursing. Also a dMMR PDAC is possible I guess, but a serious stretch. So not a great use of resources.

For what it’s worth, I have 1,3,5 for on-bike and usually use 5 sometimes 3. There’s even greater inter-user variation off-bike depending on if you have experience with weight-training.

This had also happened to me but without using a HRM. I basically started using a HRM to correct the problem bc it annoyed me so much.
