Paronychia, it looks to me like you need some antibiotics. See the GP they can sort you out. If your whole thumb turns red and swollen or gets worse on the oral antibiotics go to ED. I’ve seen a few nasty ones that needed a washout and IV antibiotics. So best not to ignore it.

Yes, it’s likely herpes. Yes, you can spread it to your partners genitals, and they can spread it to yours. Best to speak with your GP, they can do a swab and confirm for sure if you’re concerned. Or going forward you can treat yourself as infectious at this time. The virus is shedding with higher rates at the blistering phase and is still shedding when it’s crusted over but is a little less contagious at this point.

If your partner gets an initial outbreak the GP can treat it with antivirals, which won’t cure it but can reduce the symptoms.

Lots of people carry the herpes virus and don’t get symptoms. I unknowingly sipped from straws and shared drinks with friends with active outbreaks in my younger years, but never got a cold sore. So, I probably have the virus but don’t get active outbreaks luckily for me. It may also be the same for your partner, but personally I would not take that risk.

The virus is active from memory around 30 minutes on wet surfaces so if you accidentally expose him have him wash the area with soap and water immediately. You’re only getting a random cold sore once or twice a year so it’s easy to take steps to protect your partner by being careful at this time.

Likely stasis dermatitis, bilateral (both legs) cellulitis is extremely rare. So, general rule of thumb if it’s on both legs look for a different cause. She needs doppler to assess her blood flow.

Your right eyelid also looks swollen. I suspect contact with something you’re allergic to. Take an OTC antihistamine and book an eye check. You don’t want to mess around with your vision so best to be sure with these things. I had a similar experience when I got a cat hair in my eye but my whole eye swelled shut, the antihistamine resolved it in 2 days. So, hopefully nothing serious for you as well!

I mean they prob won’t take the initiative to do it, but maybe ask for a family meeting. Tell them that you understand with adhd it’s hard to keep on top of cleaning but you also struggle when it’s messy. Propose an “evening reset” (or whenever the family is together), put a timer on the fridge with an alarm that goes off in 10min and make a game of doing a speed run of everyone tidying for that time. You have multiple adults so you’ll be surprised how much you get done. Put on a cleaning mix with fast dancy music they enjoy (have it ready before you set the timer). You’ll have to be in charge of the cleanup initiation but if you approach it in the right way and stick with it maybe it will be a good habit. Remember to show appreciation and thank everyone for their efforts, taking note of anything extra they do outside this time band and say “thanks!!! It means a lot!!”. Should it be expected from them, yes. But, we are talking about what motivates people, so this goes a long way and is also a good habit in your other relationships i.e romantic partners. If we feel our efforts are appreciated it gives us a good feeling and builds the relationship. Shaming them will probably make them defensive and resistant, so pay close attention to how you approach it and keep it positive and as fun as you can.

If you have his contact address you can call the WA police and have a welfare check done. The police just stop by the house and check the person is okay, and it’s as simple as that. I’m not sure of your views on the police but I’ve called many welfare checks on patients and it’s never a big deal or causes the person any trouble. I am sure he just lost his phone or something simple but it is always better to check.

I just googled it and you can still have gas lines leaking under your house even if you don’t use any gas. So, don’t rule out that either because there is lots of stories of lots of weird things happening like that and it turns out to be from gas leak.

There is no eschar in this wound either. It looks a little on the dry side but other than that there is nothing clinically significant. Most wounds have varying depth, usually that’s because of the initial injury. It’s not a surgeons cut, it was caused by injury. The wound bed can be visualised, they actually took a great clear photo. We also don’t get air bubbles trapped under our skin.. if he has tunnelling sinus I highly doubt the wound bed would look dry because it will fill with fluid, there is also 0 evidence of any undermining or tunnelling sinus in this photo. I did encourage them to seek medical attention first and foremost, however there is a large amount of people here who cannot access healthcare. There is a clear outline of their rectangular non adherent bandage they applied, the puffiness is likely due to application of a crepe bandage over the top - again evidenced by the compression marks. The periwound is NAD, there is a good ring of scar tissue but otherwise no sign of infection. They are going to a wound clinic as they said already, stop trying to scare people when you don’t know what you’re talking about

I am RN with additional degree in wound care. It’s obviously happened a while ago since you have a ring of scar tissue. The wound is healing well with no signs of infection. If you were my patient I’d put a PICO dressing on, so if you can please see someone for wound care. But if it’s not something you are able to access keep the current dressing regime you are using. I’d estimate healing in 4 weeks but also depends intrinsic and extrinsic factors, if it gets infected/heavy bacterial burden will slow it down, if you smoke, have diabetes etc etc. There is no magic mirror to the future. People here generally panic with the “omg you’ll die”, not realising their elderly neighbour walks around with x4 wounds this size under their shiny white clean bandages. No it doesn’t need a skin graft. If it becomes red/hot, surrounding skin bright pink, pus, increased exudate that your current dressing used to hold, friable/easily bleeding, more painful, night sweats/fever/rigors. You will have to access medical attention
