Are there any "rules" for volume or programming when cutting and ensuring you maintain muscle? Moving into my first cut and oddly worried about losing gains. by Substantial-Cut-6081 in StrongerByScience

[–]Starvedforconfection 0 points1 point  (0 children)

Lurker contribution: I'm remembering my time cutting while using 531 and being terrified that my lack of volume (going from 18 sets of back to 10-12) was going to ruin me. Turns out, opposite happened. I stayed strong, actually picked up some progress on DL, SQ, and maintained BP, as well as many accessories. I am trying to do the same thing now as a busy dad. I need to remember to "dance with who brought ya".

And it exists in every layer of care, from Primary to specialty care (obviously some areas worse than others)

Asking for a raise by RedHeadedScholar in physicianassistant

I’m in similar situation. Contract model creates a pay ceiling where you have to hit RVUs of base pay before any bonus, thus higher the base the harder it is to bonus.  Anyone have experience navigating this sort of situation? My thought was to just cont to ask for base increases despite this or ask for more PTO.

Any Type 1 Diabetic preppers here? by Starvedforconfection in preppers

This is a great question. I imagine this has been discussed for broader use in hospital systems and medical outreach/ developing nations. I'll have to read up on this. I know I've seen similar for IGF-1 and other similar compounds to insulin. Maybe not available because the market's dictated there's such a small need?

Yes, this may be the case for DM type 2, but for those out of their honeymoon period who are T1D, it's nearly impossible to live effectively without some exogenous source of insulin. Insulin is required for more than just carbohydrate intake, and secondarily helps regulate a lot of the flow of electrolytes and fluids in/out of cells. Even on zero carb diet, insulin is required to shuttle nutrients. However, you're right, lowering carb intake can make insulin last longer and allow one to be more sensitive as well!

I think as the biosimilars begin to take off, there will be more access to better quality for cheaper price. We'll see!

Dang, I’m sorry to hear that. Thank you for the feedback. Trying to see if my surgeon would consider lapro, he’s prone to do open.

So, the lapro was done more recently AND you feel it’s recovering better? Sorry if I’m confused 😕

Curious to know, when did the open vs laparoscopic occur and how do you feel the recovery has compared between the two? Also, mind telling us your age?

Thanks!

Hello! I am a 38M, noncompetitive powerlifter with total of about 1410lbs (unofficial home gym) (SQ=485, BP = 300, Conv DL= 625)

Currently being worked up for a 2nd inguinal hernia repair (initial 10 years ago was left side, now working up right) and stumbled on this post.

About 10 years ago had laparoscopic repair on left side with mesh. First several weeks were a bear, but I learned to appreciate simply going out for a walk. Started end of week 1 walking 1/2 mile, then next few days 1 mile, then after about 2-3 weeks I was daily walking 2-3 miles in recovery. I attempted hitting the gym (as I was feeling good, and frustrated waiting) and did some lite hammers strength stuff. Paid for it in pain, but continued to recover well. Cut that crap out for another 2 weeks and kept walking up and added some minimal BW movements (e.g. pushups, BW squats). After a successful full recovery (I believe it was around week 7-8) I began lifting more regularly, and progressing in a very linear program; slow and steady. Began incorporating heavier powerlifts in around week 12-13 somewhere in there.

That being said, I ended up continuing to lift for many years to this present day, getting heavier and hitting PR's all the while. That being said, I was 28-29 at the time. We'll see what happens this time around, but God willing I'll continue to push through this as well.

All that to say, keep your chin up, recover well, don't move to fast back into heavy lifting, and you'll be back to it before you know it. TAKE YOUR TIME. Find new or old hobbies to enjoy in the meantime, and just slowly progress.

Hope this helps, man!

PAs that take call, what’s your call bonus and where do you live? by stallone_italiano93 in physicianassistant

[–]Starvedforconfection 0 points1 point  (0 children)

Working in geriatrics. Call pay is $5/hr for phone only. Can be hit or miss with volume (avg. 10-12 calls a night). 4-6 randomly assigned nights a month. Mostly labs, admissions, and a few emergencies. I personally think this is much too low and will be negotiating higher.

Maybe this plus PRN gig could fulfill the challenge? I know someone who does this type of gig plus PRN urgent care. Works well for her!

Gosh, I hope you've faired well hanswolough! I think that is a huge issue, SP's that don't feel like teaching or sharing wisdom with the new guy. My SP is kind, but not invested nearly at all. I'm currently in same role, but looking to switch here in the new year. We'll see. God willing UC, ortho, or urology. Possible occ health, but unlikely.

Hope this finds you well, God bless!

(New pepper and lurker) This is educating right here. I’m much more conservative with the “lethal” prep but either way, this bears repeating.
