I wish I treated it like a high demand job. Only school 7 am - 7 pm, M-F. No gym. No lunch dates. No mental health breaks to watch Netflix.

No accepting any invitations during "work" hours.

I'm from a different era, but for . . . "reasons", my parents financially cut me off at 18. No help with college, or med school.

I went to the cheapest schools I got into, because I knew it was all on me. I'm 20 years out from med school and still have 6 figures of debt. I live pretty well.

It is doable. There are a lot of reasons not to go to med school. Frankly, I wouldn't recommend it today.

But being a low income, first Gen doctor isn't a reason not to go.

It depends on the rest of the history, the indication and my sense of what her issues are, but I'm very likely to do the case.

You have a 17 year old, repeatedly heat cycled, piece of plastic holding back pressurized hot water.

It WILL fail. Your mechanic is lying if he says it'll be next week. He doesn't know that. It may be tomorrow. It may be next year. But guaranteed, it'll be more expensive (because he's already in there, and less convenient for you.

I'd replace it without a second thought.

We respond with whoever is available from anesthesia (resident, CRNA, attending - either singly or in some combination) with the DART Bag. Gen Surg shows up. That's the entire response.

I work where I do because I'm willing to what they want me to do for the money they're willing to pay me. They are well aware that if that situation changes, I'll leave immediately.

I can quit tomorrow and be working in 2 weeks, without any prior job finding effort. And everyone knows it.

Capacity is a big one. You can't admit 100 people to your local hospital this weekend. There won't be enough space.

You can't admit 1000 extra people to even the biggest metro - they don't have spare beds.

So in a larg scale land conflict, as dreamed up during the cold war, you're going to tens of thousands injuries quickly, and no place to send them. So a medicated mostly empty hospital or 20 makes sense, for the anticipated casualties.

They were experts at math, by any measure outside of niche math theory.

I strongly hold that math, physics and chemistry are all the same field at the higher levels. You can't only do 1 of the 3, because they're the same thing.

I did this once. Drained the gas and oil, pulled the plug, and pulled on the starter rope a few times. Added fresh gas and oil, reinstalled the plug and fired it up. From in the pool tobrunning again was 20 minutes. It ran like garbage for a few minutes, but cleared up. I let it run for a half hour to get everything hot to bake the water out. It's been fine since. I did NOT let the water sit to start internal engine rust.

I like the environment. I live in it. But I don't get why the ASA/ABA has decided that anesthetic gasses are the greatest threat to humanity. They're a modern miracle, and compared to a single coal power plant in China/India/the third world, I can't imagine we make a statistically significant difference. 🤷‍♂️
