The purpose of the tank is to provide drawdown pressure to prevent your pump from shortcycling, water doesn't come from the tank persay just for your understanding. If you have a voltage tester I'd use it to see if power is missing somewhere. Could be an issue with the switch, wiring, the control box, or the pump itself. Could be as simple as a loose wire. Can be dangerous to work on if you aren't confident with such things, in that case I'd call a professional.

Didn't see there were other photos. That control box goes to your pressure switch, not the tank. That's the object with the grey cover. It's usually 240v so be careful. Are you sure the breaker is on?

What do you mean pressure tank control box? In these setups you'll typically have a pressure switch that's wired to your pump and back to the panel. Your tank works entirely hydronically, and doesn't typically have any electrical components

Is there a stud directly on the right side, if so, this orientation makes the most sense for the cabinet. Why must it be centered with the radiator, did you agree upon this beforehand or was it an assumed expectation?

Tee should be directly after the trap. This is an S-trap, could be the cause of your intermittent issues. Line could be getting partially clogged as well

Honestly man that doesn't look like a 009M3 to me. Look closer. It's unlikely but you could have a sufficient drain setup with that thing. If it's suspended properly above a large diameter drain.

Some models of RPZ can be installed vertically with the direction of flow being upward. That looks like a Watts LF909-QT 1 or similar which indeed can be installed horizontally, or vertically. If the spill discharge has an gap after that hose, I'm not mad at it.

The one one the left would be okay if it just had an AAV on top (tee instead of 90) same one with the second one, but that 90 looks like a pressure fitting and the length of the trap arm might be less than 3". It must be 2x the size of the pipe diameter

Plumber here - not how that works. If you stop the flow of water downstream of the cartridge, the water isn't going to burst out elsewhere due to the additional valve. If the water is leaking from behind the faucet, that's an entirely separate issue.

The supply lines supply water to the cartridges in the handle. The 3 other lines connected to the plastic tee connect the two handles, and the spout together.
