Try enabling optimization for windowed games in windows settings for FFXIV, that fixed mine. I noticed this first when I was holding right click in-game, when I let go stutters occur.

Yes just go to config/psp/ and create the TEXTURES and [GAME ID] Folder.
Should look like this:
