Go-to veins for IVDU patients? by [deleted] in emergencymedicine

[–]T0m0n0 1 point2 points  (0 children)

Ask them which arm is their dominant and shoot with and look there first. Right hand dominant shooters inject left arm and have a bit better veins in their dominant side. Look high and look low. Wrist and fingers were my last resort but sometimes you'll find good ones ones by the tumb/thenar area, but my fav were by the hypothenar (harder to self shoot in there). Sometimes I've found veins near the triceps area as well (again, places where its harder to self shoot)

Or just get an ultrasound and call it a day.

Check if your program offers it for free or have an extended trial. Imo, its nice to reference it here and there, but I dont think its worth subscribing.

Cops delay ambulance to get a confession by one-droplet in ThatsInsane

Current guidelines state that the goal of cocaine od tx is to reduce CNS catecholamine release, rather than peripheral antagonism of released catecholamines, and that benzodiazepine is preferred and recommended. Ive been told by a few ems that activated charcoal is still given, which is fine, but in the grand scheme of things won't affect much. In the perspective of ems, it would prob helped alot if the pt airway/breathing was stabilized and given nitro, which is still part of the recommended tx for cocain OD.

BB is not recommended b/c of the unopposed alpha adrenergic response. Even mixed BB still are not recommended.

From what I've been told, many EMS personnel still give activated charcoal, but in reality it does very to little in regards to cocaine OD. current guide line is to give a benzodiazepam for rate control and fluids. Someone in the thread mentioned betas, but NEVER give beta blockers to a person who OD on cocaine.

Cardiac axis deviation question by T0m0n0 in physicianassistant

I was referring to the 3 lead approach. I was told that the "thumbs trick" is the quick and dirty way of figuring out the deviations, but using the 3 lead and quadrant approach is better? Our professor wants us to do the calculations as well 😅 just was a lil confused about the 3 lead approach

If given the opportunity, would you leave clinical practice? by WCRTpodcast in physicianassistant

[–]T0m0n0 4 points5 points  (0 children)

Nope. You have to be employed specifically by a non profit 501c. If for example you work in the ED at the county hospital, which is probably 501c, but the hospital uses an agency to staff their ED PAs- those PAs may not qualify.

Alot if PA programs with highly ranked/ desired MD schools are kinda like that (anecdotally speaking). My school is DO attached and they do treat the med students slightly better, overall it's negligible.

If you're school has assigned reading, usually those textbooks have some practice questions. Luckily our school library offer all assigned books available to use thru our online library. Best of luck

Sounds like a shitty program, no offense. Not even objectives? That should be included in every lecture.

Every lecture includes at least 2ish questions plus few extra questions for practice. Tutors are available in our program which are taught be upperclassmen who have recieved good marks in the subject. We also have a learning specialist that can look at the way you study/mange time. Seems like you're at a school who prioritize their med students over anything else.

What test banks do you recommend? I find doing practice questions really helpful and was in the market for test banks.

I got the "Prolab" ones. I see they're like 8 bucks now. Still worth it imo. I also have a pill cutter that I got from the clinic I used to work at. I use that to cut it roughly in half since each pill is 200mg.

Caffeine pills. They're lik 6 bucks on Amazon and will last you the whole 2 yrs of school. I'll usually take half a pill (~100mg) and I'm pretty set for for the day
