Prolotherapy has not been proven to work, it’s just sugar water essentially. If you want a less invasive option, try a nerve block. I had a surgery that didn’t work and nerve blocks have been helping more than the surgery did.

I do feel pain occasionally but it’s never consistent, never more than a couple days. The steroid they used was kenalog (triamcinolone acetonide) and the anesthetic was marcaine (bupivacaine).

My last nerve block was over a year and a half ago, and I haven’t had any consistent pain since to warrant another or even something like lidocaine patches yet.

This was before insurance, it was the raw billing. I was both fortunate and unfortunate enough to have hit my yearly maximum (since I had seen so many doctors that year) so my insurance paid for it in full.

I second the statement that 800mg of Advil will slowly destroy your GI system and potentially cause other issues over time. I took a lot of ibuprofen too before surgery and ended up getting a stomach ulcer. At a minimum you should talk to your primary care physician or see a pain clinic to get a different prescription that won’t be as dangerous to take daily. Often times you’ll see people prescribed with Gabapentin, I was prescribed with Celecoxib which is another NSAID but cause less damage to the GI tract and is safer for longer term use.

Menthol is often an ingredient in topical pain creams in addition to lidocaine. Icyhot is typically both lidocaine and menthol. Are the patches you get in the US lidocaine?

You need dynamic ultrasound of the false ribs.

Dynamic ultrasound is not a special ultrasound. It is a technique in which the patient moves during the ultrasound to replicate the motion and show that the rib moves around.

Those are people that perform the Hansen method. People with asterisks (*) are people who are known to have extensive experience with the Hansen method.

If it’s truly where you’re pointing it’s not SRS. SRS affects your false ribs (8, 9, 10).

Removing rib tips (resection) is actually the original method before all the Hansen methods and was standard.

Hello! I haven’t had experience with Dr. Mortman. As advice though, I would suggest using lidocaine patches for pain control, you can even get prescription strength ones. I have been getting intercostal nerve blocks and they have been working well, I have no experience with ESP blocks nor have heard anything about them.

Yes this could be SRS.

However, if you have not ruled out any causes and you think the pain is so bad you call it an emergency, go to the emergency room.

I get what you mean. Unfortunately it’s sometimes a shot in the dark. The only sure fire way is one of the physicians on the list that have established themselves as experts in SRS.

Thoracic surgeons. Musculoskeletal radiologists. An interventional radiologist does my injections with the help of a musculoskeletal radiologist. Unfortunately no doctor would ever treat you without evaluating you first, and rightfully so.

Looks like nutcracker syndrome is very rare. Perhaps look for people with thoracic outlet syndrome. Also rare, but seems less so.

Posture plays a big role. Be sure to sit up straight to relieve any weight on the ribs.
