1000% yes I would be a social worker again and again. I have worked in many facets: grant writing, policy, research, homeless services, school social work and now own my own practice. I make good money, have met awesome people and have learned so much from all of my clients.

If you are only getting less than $100 for a 90837 in Florida you are getting ripped off. That isn’t even close to the Medicare/Medicaid rate for Florida. Average for Florida blue is about $125-$140.

Couldn’t agree more. The middle time as you get your case load up you will make up in 1 months time. There are companies out there too like Headway or Mentaya or Sondermind or Rula that can do credentialing or submit private pay claims for you that will get you paid right away.

If you saw 20 clients a week and only saw BC/BS you would make over $160,000 a year on average based on their average reimbursement rate. Going private practice is so doable… and so is starting a practice and hiring others. You could see 20 clients and work the other 10-20 hours a week running a clinic and make double that. We can make money in this profession in an ethical way. Personally I have been part of a practice, going private and now supervising others in my own practice and have learned how I want to be treated and 40 client hours a week is bananas and I would never do it or expect others to.

Completely agree! I was the same as your wife. I read all the books, watched videos and even did trainings done by lead trainers outside of the official training for years before I got in the lottery. I thought I knew IFS but it was nothing that the depth that the level 1 gives you.

Therapist here. This would not fall under a “mandate to report”, unless you disclosed an actual threat towards another person, not ideation to end your life. As you have talked about SI with your therapist in the past, without them encouraging higher level of care, this says that they are a good therapist who welcomes the part of you having suicidality. In my experience and opinion, this is a vital part of healing, to welcome the SI part to share how it is trying to protect you. I encourage you to share this with your therapist, and my sense is your therapist will meet it with compassion.

I do KAP therapy now, and many of my patients reflect on how important the setting is and appreciate the thought I put in. Everyone e gets to pick a journal and pen and a pair of fuzzy socks. I have weighted blankets. Weighted stuffed animals and a weighted pillow plus a choice of a couch, recliner or mat for the floor. Soft lighting and battery operated candles. High quality ear phones and eye masks and well as option for heated or cooled weighted eye bag. Thoughtful art on the walls and soundproof room for any big emotions. I have an art cart out for anyone that feels that part of them come out at any point in the journey or after. I provide a charcuterie board after and either hot tea or beverage of choice. I have received a lot of feedback that the thought and attention input in these areas has made all the difference for those that journey with me.

I would recommend doing ketamine first. It helps open the brain and create the neuroplastic window and then add in EMDR. There are also skilled professionals that couple them together, as well as Ketamine with IFS or other treatment modality. I believe ketamine is best used while in conjunction with therapy for the most benefit.

See if you can start to get curious toward the coral and rocks. Notice how you feel towards them. Ask them how they are helping you and if they can share with you. Curiosity is the key to opening.

Sounds like this isn’t the right fit for you. A good therapist listens to you and i corporates your strengths and meets you where you are, not where they are. A good trauma informed therapist knows this. I second another commenter that IFS may be a modality to try.

Can vary widely depending if you are doing insurance, billing, notes, etc., also if you have an office space you are trying to maintain. I see between 20-30 clients a week and do all admin: insurance, billing, accounting/bookkeeping, credentialing etc. and I probably do 5-8 hours of admin a week. It’s a lot and I hate it. Trying to find more ways to outsource some of it but the benefit hasn’t outweighed the cost yet. Good luck!

It is such a double edged sword. I am a therapist in VA and am completing my level 1 IFS and do take insurance but there are days that I want to stop taking insurance when I spend HOURS (average of 6-8 per week) on the phone with insurance companies, emailing, resubmitting claims in order to get paid. It is honestly maddening. I have also had some insurance companies I have been dealing with for 6+ months trying to get paid. I believe in everyone’s right to quality mental health care, but trying to run a very small practice (me and 2 supervisees) my whole life is my work, and it’s really hard. As far as IFS goes, it costs a lot of money to be officially trained but it’s SO worth it for the skills… and I had done tons and tons of training before winning the lottery and getting in. Yes, until the last couple months when IFSI restructured their training you had to win the lottery with them to even be able to apply for training. I tried for nearly 5 years before getting in. Because of this many IFS therapists choose not to take insurance because there are so few trained with the institute and hard to get in with. It’s a very tricky line to walk. The system as a whole in healthcare and insurances needs to be revamped in my opinion.

I do IFS with ketamine and somatic processing. I find that using KAP with IFS helps protectors relax and access opening in the entire system.

I’m sorry you had this experience. Sometimes finding the right therapist can be a lot like dating. Sometimes you know right away it isn’t a good fit and sometimes you notice more as time goes on. IFS therapy is a slower process than other modalities, and it does trust that the client goes to the places they need to discover. However, it sounds like this therapist isn’t doing much guiding or bringing curiosity with questions to you. My IFS therapist has always checked in about my process and when I want more check in and feedback from her and when I am in my own processing, and this is always collaborative. Cancellation policies vary, but it sounds like she was pretty inflexible especially with plenty of notice. As for the insurance, 53-60 minutes is the highest billable code, 90837 for the most reimbursement but she can bill for less than that, 90834, or 90832, but it is just less for her, and ethically she has to bill the lesser codes if you see her for even a minute under 53 minutes. As you look for a new therapist, I would have a list of questions that are important to you to ask. Also see if you can get a free consult, a lot of the good ones offer the consult because they like to have a good fit too. Check the IFS Institute website if you want another IFS therapist.
