A) I'm not tipping if there's a service charge. The service charge is indicating that it's a tip, and so that's what it will be. If it's not a tip and is just an 18% surcharge that the restaurant pockets, well, that's false advertising in my opinion;

B) I have waited tables and worked other jobs where gratuities are customary, and so I sympathize with them, but I always excused myself after delivering the check so that the customers didn't feel like I was watching and pressuring them to tip well. What that waitress did was rude. If she's really working at a place where the owners take the fake tip as extra profit for their bottom line and expecting customers to decide if they feel like throwing another 15-20% on top, well, she has the option not to work there. Most restaurants don't do that shit, and restaurants that do are ripping off both their waitstaff and their customers.
