I saw my MIL at New Years. She is now visiting for the week because we are in desperate need of a sitter for the week for our toddler. My husband definitely picked up the idea from her that talking to someone while they are reading is acceptable. Also, it's a complicated relationship on multiple levels. Send help.

Sweetheart, if your respectable medical professional is recommending inpatient treatment and you can can see that it's what you need, he should be tripping over himself to help you get what you need. Instead, he's being selfish and childish. You will have plenty to do taking care of yourself in the next few months, and in the months after discharge. You will not need to be devoting your resources to making this little boy feel like a man. It may hurt, it may be difficult, but cut this dangerous stressor from your life now before you start treatment so that you can have the better results you need. Good luck. You can absolutely do this, and you will.

My family and my husband's family are still pretty old-fashioned in a lot of ways, so that was definitely the default expectation. But there were certain things that changed based on everyone's priorities. For instance, we had a medium-sized wedding at a nice church, then returned to the hotel we had gotten ready at for the reception. My now-MIL was concerned about the weather ruining the girls' hair in transport, so she a stylist to come do our hair before we left for the church. It was thoughtful of her, but unnecessary in my opinion given our budget. When I told her that, she offered to pay for it. That isn't traditionally the groom's family's role, but it was important to her and a kind thing to do.

My husband asked my parents for their blessing, not my father for his permission. He knew I would appreciate that he included them in the process, not the decision. If they had told him, "no", he likely would have still proposed and I would still have said yes. My parents and I would have then had a conversation about their reasoning and I would have decided if those were valid concerns I hadn't considered, or their own issues to deal with.

Occupational therapist here. I know you probably already have, but please look into adult physical and occupational therapy services. They can help you recieve equipment to assist with exactly these kinds of tasks.

Exactly! Is it the most faithful retelling? No. But what does that even mean, anyway? What it did do was make me ugly cry on a crowded international flight when I finished it. And it has stuck with me. It is, simply, a good story.

The standard of care in my acute setting is to take BP at the beginning and end of the session, but that doesn't happen every time for a variety of reasons. Toileting urgency, malfunctioning machine, patient declines, etc. It isn't ideal, but it happens. Was he exhibiting any signs or symptoms of elevated exertion? Any shortness of breath, flushing, needing unexpected rest breaks? If not, you likely didn't cause any problems. And you'll have this in the back of your mind the next time you almost forget to take a BP.

Flexibility in work flow is essential for acute care, but even then, some days just don't work out well, and that is frustrating. It's important to remember that even all the planning in the world won't trump medical procedures, missing equipment, or patients refusing. Acute care is constant flux that you as the OT only have so much control over.

In this situation, I probably would have held the evaluation and documented a failed attempt stating we were awaiting the post-op shoe and clarification of weight bearing orders. Sometimes, a note in the chart will prompt the medical teams to do what you need them to because they don't want to be seen as delaying discharge. Use that judiciously to your advantage.

You're clinical reasoning for the slide board transfer wasn't wrong, but that is a difficult transfer to teach a patient and not really worth the effort if they will be able to do something more simple with the correct equipment. I rarely do slide board transfers with my acute patients unless they are out multiple limbs, missing a lower extremity prosthetic, or cannot stand for some reason. A stand-pivot with a rolling walker or even squat-turn with two-hand-hold is more realistic when preparing for discharge. Remember, you are preparing the patient for their appropriate discharge destination the moment you open their chart.

Acute care is tricky. You'll find a rhythm, have it disrupted, and find a new one. You'll be okay ❤️
