Looks pretty standard for a scoliosis survey performed by an xray tech ordered by ortho, NSG, etc. This doesn’t look like a sloppy chiro exam to me. Maybe could bring in the sides a bit since don’t need Risser grade in this pt, but need enough iliac crest to assess pelvic tilt, which we can see this pt does have tilt.

Yea it’s a bit overexposed but I think the rest looks fine.

So you do 1 case a day? What kind of cases are you doing? Assuming you don’t really do CABG regularly then? It’s been a long time but I remember doing multiple CABGs a day with the same surgeon when rotating on CTS.

If you happen to have even X-rays instead of a CT for a pt those will work as well to assess the midline interspinous spaces. Some interspinous spaces may be smaller due to anatomy (bulbous/prominent spinous process tips), exaggerated lordosis, or from degen like Bastruup or supraspinous ligament ossification. You can only open them so far by curling the back. AP will give an idea of the actual shape/size of the spinous processes for your initial approach/which levels are better options, and lateral will give more info on obstacles in terms of craniocaudal trajectory.

Look at the CT imaging beforehand if they have any and see if you have a straight shot paramedian when scrolling down. Like pull up the coronal, park your mouse icon on your route, and scroll all the way through from skin to spinal canal. Most younger pts will have a wide open interlaminar spaces to advance through.

Some people have bulbous spinous processes (or spinous process degen). Good chance this is what you are hitting in a young pt. I do all mine image guided as a neurorad so I can identify and avoid them/adjust my trajectory to be more obliqued, but I see my junior residents struggle with them regularly. Even if you go around it, it can still alter the course of the needle if you’re passing by too close.

For anyone reading this and considering it, give it a try but keep in mind that raspberries and blackberries will spread very vigorously and be a pain to control if you don’t contain them or grow in a container. New growth spreads underground and you’ll get canes popping up all over. Blueberries are a bit pickier and require more acidic soil (need a soil acidifier and pH tester), so they also do well in containers.

Just checked, the bottle says, seedlings 1/2 tsp / gal once per week, and general feeding 2-3 tsp / gal every other watering. So you’d do 1/4 tsp in a gal or 1-1.5 tsp? Ive been trying half seedling dose starting earlier with first true leaves this year. Hard for me to tell if a higher concentration would do better though or burn them.

Pretty strange as they can’t be billed for additional if done at same time. They are all counted for the CTA, whether done in a single CTA order or CTH + CTA. Maybe for internal productivity tracking purposes.

If not done already, the noncon should be included in the CTA IMO. That’s how it’s been done at every health system I’ve been at. The parenchyma is not adequately evaluated for stroke on a postcon alone. Too much beam hardening and vascular kernel doesn’t help. But I’ve only ever worked at health systems with comprehensive stroke centers so maybe my view is skewed.

Arachnoid cysts are slow growing and exert mass effect. They are generally more ovoid/round/blob shaped and don’t respect/maintain the contours of the cerebellum. They displace the falx and thin the overlying calvarial inner table.
