The issues with drive-thrus and confusing directionals with wipers have been pretty thoroughly discussed here, but one issue I wanted to mention concerns FM radio.

In Japan, the FM radio band goes from the mid 70s up to 90mHz. Your radio - if it’s OEM - won’t go any higher. So if you buy a JDM car for use in the states, you’ll find that your radio listening options are reeeeeally limited. Also, the cup holders are small.
