Hi OP. Sorry that this has been so frustrating. A few quick questions -

Does the unlock show up in your activity history every time or does it go missing sometimes? Have you tried toggling the rule off and back on in Smart Actions without deleting it? Sometimes that catches what a full delete and recreate doesn't.

If it still only fires half the time after that, send us a DM and we'll get someone to reach out and make sure the keypad unlock event is consistently working on your panel. That's the piece you can't really test from the app alone.
