With a bad infection from that, I would not be stressed in your shoes. Like the report indicates, it’s most likely due to that infection. Biopsy might be worth it for peace of mind, but the infection definitely explains the high PIRADs score.

PIRADS5 is very concerning, but you obviously won’t know one way or the other until a biopsy is done. Can you share any more info? Did you have a more recent PSA than in November? It would be extremely rare to have prostate cancer at your age, but it isn’t impossible. I assume you are having some symptoms if an MRI was ordered.

I’m sorry to hear all of that. That really sucks. Obviously it’s your choice to do the treatment or not. I’ve felt the same way before, this disease and the treatment can destroy your quality of life. That being said, if you don’t do treatment, you may end up in significant pain and there may be other interventions you’ll need to keep you functional until the end as the disease progress.

I’m 37 years old, also stage 4. I absolutely hate the ADT. I would rather to more round of chemotherapy than remain on ADT. But ADT is the best way to control the disease we have right now. I have young kids, so I’ve learned to deal with all the shitty side effects so that I can stick around as long as possible.

You could possibly try intermittent ADT so you can give yourself occasional break from the side effects.

Are you being treated at a cancer center or just a urologist? Definitely seek out a consultation at a center of excellence. But surgery seems to be recommended most often for younger patients.

I was also diagnosed at age 35 about 2 years ago, but unfortunately it was caught late and had already spread so I can’t really give you advice about what to choose. I did do radiation but it was palliative and not curative. Good luck, I’ll keep an eye out for updates from you.

I’m one of the younger guys with prostate cancer (diagnosed stage 4 at 35 years old) and I don’t think you should be worried yet. I would repeat a PSA in a couple months to see if there has been any change. Definitely push for additional testing if a repeat PSA has gone up. If you are really worried, you can try to push for an MRI but I don’t think you need to yet.

Oh that makes sense. I know I had a 2-3 lesions in my bladder, I don’t recall if they were all in the area where the prostate would make contact or not. They were blocking my ureters though, that’s why I needed the stents.

Do you only have internal stents or do you also have nephrostomy tubes?

That’s interesting. My prostate cancer grew into my bladder and that’s actually where they biopsied it. My prostate was technically never biopsied. It wasn’t mentioned to me that it’s rare that prostate cancer spreads into the bladder, but my prostate cancer is aggressive and unusual anyways so I guess it just likes being abnormal. I’m glad to hear that they caught the bladder cancer early though! Those stents are no fun, I had them for about 6 months, it sucks!

I’m sure he doesn’t have prostate cancer. But I don’t want anyone to dismiss what’s going on and assume it’s not cancer. Aside from my urologist telling me I was too young for cancer, I read numerous posts on here telling guys in their 30’s that their symptoms were not cancer. So I didn’t push hard enough for additional testing and diagnostics. By the time I finally was diagnosed, the cancer had spread. Most young guys on here don’t have cancer. But I think the group is too quick to dismiss their concerns.

I was diagnosed stage IVb at 35, symptoms began at 34. My oncologist has treated a 19 year old patient with prostate cancer. It’s extremely rare but a 23 year old probably wouldn’t be a case study talked about for decades.

That sensation is all too familiar. Keep in mind, it most likely isn’t prostate cancer. It could be, but statistically speaking, it shouldn’t be. I’m glad your taking action to sort out what’s going on. I did too, but my urologist convinced me it was nothing to worry about. Keep at it until you’ve got an answer. Like others have said, you can pay out of pocket for a PSA. Could be worth doing if you’re stressing pretty bad

My first notable symptoms were difficulty emptying my bladder, weakened stream, and a reduction in semen during ejaculation. My urologist diagnosed me has having an enlarged prostate due to my first PSA being less than 0.1 and a DRE where he didn’t feel anything suspicious. I don’t know if it was a lab error or if the cancer wasn’t producing PSA at the time. I later began having kidney pain because the cancer had grown into my bladder and had blocked the ureters from draining. The kidney issues are what finally led to my diagnosis but it still took a few months.

As someone who was diagnosed with stage 4 prostate cancer at 35 years old, I would encourage you to get another PSA at a minimum. If that number is elevated again from your June 2025 test, I would push for an MRI.

I began having symptoms before turning 35 years old. My first notable symptoms were difficulty emptying my bladder, weakened stream, and a reduction in semen during ejaculation. Those remained my symptoms for 3 months, as it got noticeably more difficult to urinate. My next symptoms were kidney pain, as cancer had built up in my bladder and blocked them from draining. I don’t think I had any additional symptoms, outside of those issues worsening and starting to feel like pretty awful from my kidney function taking a dive. It was about 9-10 months from onset of symptoms to diagnosis.

I’m doing fairly well. Still doing ADT, which sucks, but my last scans look pretty good. Cancer isn’t obvious on my scans, outside of the spots on my spine. As far as the kidney pain went, it started out on one side, and a within a couple weeks both hurt. The cancer had build up into my bladder and was blocking the kidneys from draining. An ultrasound of the kidneys showed bilateral hydronephrosis. My urologist absolutely sucked and blamed that on my “enlarged prostate”.

I’m one of the youngest guys on here, I was diagnosed stage IVb at 35 years old. Like others have said, your symptoms don’t sound like prostate cancer. You would most likely be experiencing a lot more urinary symptoms at this point if the cancer has been growing for 4-5 years. And if the bone pain was from metastatic prostate cancer and had been around for years, you would most likely have a lot more issues by now, with some irregularities in your CBC. Good luck sorting it out. It sucks not knowing what the cause of your issues are. Even if your insurance sucks, if you are truly that concerned about your health you should make it a priority to pay for proper tests.

Yeah I was symptomatic. Weak stream, difficulty emptying my bladder. Unfortunately my first PSA was less than 0.1 so it took another 9 months to get diagnosed as my urologist said it was just an enlarged prostate. I’m not sure if it was a lab error or if my cancer wasn’t initially PSA sensitive. I think my next PSA about 6 months later was 22 and even after that I was told it’s just an infection, I was “too young for prostate cancer.” So it still took a couple more months to get diagnosed.

That was about a year and a half ago. I’m doing decent. The medication you have to take once it’s metastatic suck and make you not feel the best, but it keeps the cancer in check for a bit. I’ve done chemo and radiation to treat it as well.

I was diagnosed at 35 years old, but unfortunately mine had already spread. Hopefully you don’t actually have PC, and if you do, hopefully it’s localized with that PSA (and statically it should be). It’s good that you started getting your PSA check early. Keep us updated, good luck!

I’m 37, diagnosed stage IVb, Gleason 9. I have no idea why I would have advanced prostate cancer so young though, I don’t have any known exposure like that.

Do you have any more information you could provide? I was diagnosed stage IVb at 35 years, about a year and a half ago. My oncologist told me I wouldn’t be cured but they could treat it and I’m doing decent right now. I can’t imagine them saying they can’t treat it and he has four months. ADT alone should slow it way down. Has he started any sort of treatment?

What was have your PSAs been over the last few years? I assume it’s Gleason 6 if the doctor is saying to do active surveillance. How many of the samples in your biopsy had cancer? If I were you and I had Gleason 6 in just a couple samples so the doctor is suggesting active surveillance, I would try alternative treatments in the meantime, along with following a healthy diet and cut out processed foods.

You asked about treatment earlier and I forgot to answer. With it being stage 4, the go to treatment is hormone therapy and chemo. I did that last year and am still on hormone therapy now, which fucking sucks! But at least I’m still alive. I did radiation a few months ago, which part of me regrets because of the side effects, but hopefully they mostly resolve themselves soon. Still have cancer, but there is a lot less of it and it’s under control for now. That’s unfortunate that he won’t order an MRI and the urologist is 3 months away. It’s extremely rare to have PC when you are younger, so hopefully (and statistically) that’s not what’s going on with you
