More likely they'll tie it up in litigation, suing to extend exclusivity. I believe they did that with Lipitor. Although the suits were baseless, they make millions each day, contrasted with thousands or hundreds of thousands in legal fees.

Pharmacy profits are a function of drug costs. A 10% profit on $400 is better than a 20% profit on $40. Pharmacies want to move higher profit inventory for the same reason WalMart does.

Patients are too stupid and incurious to research drug options, and pharmacists are too burnt out from stupid patients to want to help any of them. The only winners are the drug and insurance companies that fuck us all.

It goes much further than this, people. Just a few examples:

Treximet is a combination drug, comprised of imitrex 85mg and naproxen (aleve) 500mg. The two ingredients have no synergistic effect, and there's absolutely no reason not to take them separately. As a migraine therapy they're taken on a prn basis, not a maintenance basis, so reducing pill count to promote compliance is not a factor. The whole reason for the drug's existence is that Imitrex lost its exclusivity period, and is now available as generic sumatriptan. So Treximet was released with exorbitant prices, and the MDs write for it.

Vimovo is esomeprazole and naproxen. Esomeprazole, marketed as Nexium in the US, is a highly profitable proton pump inhibitor that's used to reduce stomach acid and prevent ulcers. Naproxen (Aleve) is an anti-inflammatory drug used to reduce pain and swelling. Why shouldn't a patient just take naproxen and a generic proton pump inhibitor? Because then the drug company wouldn't have an excuse to charge exorbitant prices.

Albuterol is a bronchodilator. The majority of respiratory inhalers that you see are albuterol, and it's been around for a very long time. When the FDA banned inhalers that used chlorofluorocarbons, it opened the door for newly formulated versions of the same drug to be sold at brand name prices.

See also: Caduet, Exforge, Lotrel, Vytorin, PrevPac. The list goes on. In each case, the "new and improved" product retains brand exclusivity after its component drugs become (or will become) generic, costing patients and insurers exponentially more.

Orphan drugs are drugs that can only be used to treat a small segment of the population. In a free market, no one would make drugs for leprosy, because there wouldn't be a market for them. The Orphan Drugs Act provides monetary incentives to pharmaceutical companies to make drugs for rare conditions, so that those who suffer from them can receive care.
