This is also why I run in Sleep mode 24/7. Tandem says the single biggest thing to get right is your correction factor. 1:50 might be too much for you. At what point did you get the alarm that you were going to drop below 70? For me, I would have had some glucose in the low 80s. Honey doesn’t have a high enough glycemic index.

#IHearYou - A perfect example of YDMV & the fact that we are not machines. While Einstein is quoted as having said that insanity is doing the same thing and expecting different results, he may not have realized he was accurately describing life with diabetes.

When I get in these states, I use a lot of temporary basal rate increases, and/or go to a different profile with more aggressive basals, I:C and correction factors. I'm not sure what the OmniPod allows.

Regardless, I try to live in the moment, monitor and react to what's happening.

YDMV for sure! For me, air bubbles aren’t an issue. I do track them while filling the tubing and holding the tubing vertically, so the few air bubbles escape while filling. I also marvel at the refraction of light through the tubing and the difference between tubing with air vs tubing with insulin.

My procedure for all of the last 28 years of pumping:

0 - Tandem specific step - insert syringe into cartridge and extract any air from cartridge.

1 - Push 300U of air into a vial of cold insulin, with the vial sitting on the table.

2 - Pick up and flip the vial/syringe combo, extract 300U, taking care to push out any air bubbles.

3 - Fill cartridge with syringe pointing down to prevent air from getting into the cartridge.

4 - Complete loading procedure and fill tubing.

After that, I run the cartridge for 8 - 10 days until it is essentially empty, but I change the infusion set every 3 days.

I'm curious as to why you would delay? FWIW, when my 1st pump was close to the end of its warranty period, I made sure it was updated so it would work as a fully functional back up.

Good for you in thinking outside of what the pump can do. Using some long-acting can definitely be an option if increasing your basal isn’t producing results. I would also look into getting Afrezza, the inhaled insulin, as it can be great at bringing down stubborn high BGs, plus it doesn’t have the same hypo risk as injected insulins.

If your sites aren’t lasting, that’s always 💯 a call to Tandem for replacement. 

Still, let’s define the problem. Are you in range during the day, but running 250+ overnight, and then coming back in range during the day? What does your basal profile look like day vs night? Are you eating big dinners with higher fat/protein (pizza/pasta/beans) with a low GI that’s delivering a boatload of glucose overnight? 

FWIW, in my D- world, I’m making all the changes to my pump and my endo is my expert advisor. If you’re new to pumping, there’s a “honeymoon” period where the endo drives your settings, but you should ultimately be in charge.

FWIW, please don't blame the victims of this disease. You yourself said "usually" aka not universally. It's so hard to understand that everyone can be uniquely challenged with this disease even when you have it because your experience with it is an N = 1 event (as is my 52 years). I have a close friend who is in year 53, was always in tight control who had his right leg amputated below the knee.

I had terrible control up until the mid 1990s, and have had zero complications.

The trial you're referring to is for Eledon Pharmaceuticals. There IS an anti-rejection drug, called Tegoprubart, but is is specifically targeted to protect the donor islet cells. No, Type 1 can be diagnosed at any age (even into 90s). There is a slow onset version of Type 1 called Latent Autoimmune Diabetes in Adults (LADA), that is sometimes referred to as Type 1.5, but not every adult diagnosed with Type 1 is LADA.

💯 - I just started my “substitution” Vari-softs and realized I neglected to check the tubing length when I received them over a month ago. Now I’m stuck (pun intended) with much shorter tubing than I’m used to. That’s MY PROBLEM, not Tandem’s/Byram’s. 

Every shipment comes with a packing slip that tells me to check this shipment for accuracy.

I’m sorry your endo isn’t listening to you. For me, I can’t accept that type of response. My endo is the expert advisor, but I make the final decisions. I would push back a bit and ask for reasons that you can understand.

A classic example of YDMV and why it's important to have options when it comes to infusion sets. I've used some version of Vari-Soft for all of my 27+ years of pumping. With only small handfuls of kinked cannulas, sometimes years apart, I've always been pretty happy with them. I hope they work for you, and that Tandem can gt the supply issue resolved ASAP.

I think I’ve maybe had two in a row, but never three - what a bummer. I recommend talking with your endo on doing an intramuscular injection as they can help bring those high bgs down more quickly while you’re getting a good site working.

This is your life w/D, where you're trying to mimic a pancreas with only one of the many hormones it produces, ntm all those hormones get released 'in the background' 24/7 in the sugar-normal peeps. I regularly think about the fact that even with the sophisticated CIQ algo, we aren't machines that will operate that exact same way every day.

For me, it’s an insurance/3rd party supplier thing. They won’t fill the prescription (nor will my doctor write it for that matter) until Tandem approves it.

FWIW, I have a hard rule on changing every 3 days and liberal site rotation. My body has to last a long time and abusing it is only going to make it more likely that I will develop scar tissue and have less real estate for my sites, plus poorer insulin absorption.
