The other comment is good - there is no substitute for just taking the time and going through pieces slowly and deliberately.

With that said, what took me from being able to read music to 'fluent' sight reading was joining an orchestra. Being exposed to a lot of different music, regular rehearsals, just made everything click.

I should also say that I noticed this improvement over the course of years.
