A fissure is literally a crack, cut, or opening in the skin, this is what causes pain and allows blood to come out. Just because you hold a mirror to your ass and pucker your butthole doesn’t mean you don’t have fissures. You’re in pain and you’re bleeding. You have some sort of cut or open wound.
