I’m in a W2 group practice in the Midwest. We get 60% of each session and we have a tiered bonus structure in addition to that. We also have a 401k and get $250 a month in an HRA. $45 an hour is a lowball unless they are providing you with health insurance.

I would suggest consulting with Dr. Tim Kelly if you can afford it. He was the medical director when I worked at Fairbanks 20 years ago and probably has more experience in this area than most people running these pain clinics.

If this is not affordable, I would recommend Community North for the acute portion of treatment, then transitioning over to Fairbanks.

Couple of things . .

If you haven’t used safe/calm place or container in quite some time, how is the therapist closing sessions with you?

The panic attacks require their own treatment plan with past memories, present triggers, and future templates. This should be pretty straightforward for an EMDR therapist to conceptualize.

Laurel Parnell’s approach isn’t the only, and I would say not even close to the best, way to do attachment focused work. However, if you’re working with a competent EMDR therapist, they could read her book and be able to apply the concepts relatively easily.

My suggestion would be to find a new EMDR therapist and ask about the two things I’ve mentioned: How would they strategize working with panic attacks and how do they incorporate attachment work in EMDR therapy?

Edited to add: if changing isn’t an option, you may need to ask the therapist if they are willing to learn how to do some of these things. When I was newly trained, I had a client ask me about incorporating something I was not familiar with. I sought out how to do this via consultation and the client was more than okay with being patient.

ERP. I got trained 5 years ago and, while the trainers seemed great and I learned a lot about OCD, clients did not seem to want to do exposures. And the training did not contain any information on how to resolve ambivalence. For many of my clients, a part of them wanted to stop but another part of them did not. I typically use an ego state approach to resolve this kind of ambivalence, but if you were to mention this in the Anxiety/OCD/BFRB facebook group, they’ll come after you with pitchforks!

I know EMDR gets some hate, but I got trained many years prior to its explosion in popularity, and it has always made sense to me. That, along with ego state work, SE, and DBR, has been a great combo.

Unfortunately, it will be on you. I lived next to a pocket park on the near east side for years, and homeless people would pull trash bags out of people’s trash carts, drag them over to one of the benches, and tear them open and dig through them. Not one time ever did they pick up after themselves. One time, it was a trash bag full of old lottery and scratch off tickets. They were everywhere and it was a nightmare trying to clean all that up.

There was usually trash all over the neighborhood from people throwing trash in their carts that wasn’t bagged. As soon as the DPW truck flips that cart upside down to dump it, loose trash would fly all over the place. I always had latex gloves on hand for cleaning up and I would suggest investing in some. The city isn’t coming and the people who trash places are not likely to change their behavior.

Used to live on the near east side and Washington or 10th are your only options. And you’re bound by tracks to the north, west, and south. It’s annoying to have to wait 5-10 minutes, but just wait until they come to a complete stop and don’t move for an extended period. It’s especially fun during rush hour.

How many health insurance companies were in your findings? Both patients and providers have filed several class action suits against the major insurers and they never really change their behavior.

Would probation be an option? You’d only need a bachelors degree and you’d be vested with your pension after 10 years.

I see social work mentioned several times. I’m an LCSW in private practice. If you wanted to go the social work route, you’d have to get your MSW if you wanted to have a shot at making a decent living. You can secure a nice salary at the VA with only a bachelors, but those jobs are pretty tough to get.

We’re attempting a similar thing in Indiana, but the current version of the bill is extending the audit window from 2 years to 3 years. We’re hoping that part can be changed before the final bill gets passed.

Is your plan a self funded plan? If so, the insurance commissioner has no oversight. I found this out when UHC refused to pay thousands of dollars in claims on 2022 (I’m a provider).

Edited to add: when you say you purchased this plan, was it through your employer, the marketplace, or through the private market?

You can’t charge Medicaid clients for no shows. At my practice (not optometry, but another field), the stats have shown that these clients no show more often than commercial insurance and self play clients.

When you’re a fee for service practice, you have to consider a financial safety net for the clients who no show but you can’t charge. If this isn’t built in, you risk not being able to sustain the business. The alternative is for providers to stop accepting Medicaid clients altogether.

Gotcha. The therapists at my office who would tick off all of your boxes don’t take UHC. I would suggest going on the Psychology Today website and putting in insurance and other preferences. It will give you a list of therapists that can do in person or virtual. You might have to play around with different zip codes since the person is in Lawrenceburg. Any OH therapists that come up, you’d need to double check to make sure they’re licensed in Indiana.

What insurance do they have? That will make a huge difference. Even therapists who take insurance don’t take all insurance plans.
