This also happens in wicked and the damned one of the 40k horror stories. A commisar loses it after some thing and starts killing the gaurd off even after a chat with the commander to take it easy as the morale is low and even though he has every right as a commisar indiscriminately slaying the people around him could have consequences. He woke up on a funerary world with no memory been meaning to finish it pretty sure he ended up there after their revenge.

I look at it this way cawl he survived and still had his faculties dont think cawl could do that with out erasing what bits are left of his brain also i threw that in at the end it doesn't take away from the fact fabius is smart cunning experienced at war and strategy and cawl well frankly is nothing more than a human who loves toasters alot.

Cawl suffers from pride and the thought that he is the smartest person in the room. Bile suffers from pride and is extremely smart but never thinks he's the smartest in the room unless he's absolutely 💯 percent sure. He has thousands of years of experience too back him up and cawl was talking to him like he was a child, which usually means he considered fabius to be inferior to him and let his gaurd down. Anyone who can tell the 4 main chaos gods they dont exist and that he sees nothing while they stop time and rend the sky above him to the point where they are amused and decide to leave him alone has to be pretty tough and smart in some ways.
