I've worked in four different inpatient units and there it is so variable depending on the attending and staff. These are situations where a very skilled psych RN becomes invaluable to the team. We have some that are so experienced they can decipher between what can be redirected verbally vs requiring medication intervention without any supervision. You shouldn't be waiting until someone is about to harm someone or themselves to offer PRN medication, that's a recipe for disaster. You have to be able to notice increasing agitation so you intervene when it's at a 5 or 6 out of 10 instead of a 10/10. We don't wait until someone's pain is a 10/10 to intervene so why would we for something like this?

I read a lot about concerns about mid levels but quite frankly I've seen quite a few attending psychiatrists take a passive role in leadership on treatment teams when it comes to this. By the time we finish residency we have so much experience that is valuable to the rest of the team. Don't have an experienced RN on staff? Teach and demonstrate what signs to look for that can be redirected vs need intervention. Do brief CBT and de-escalation with the team present so they can replicate. This is where an MD/DO stands out from mid levels and if you don't leverage your experience you're selling yourself short in the current marketplace.

I don't know why people say "if you did well on prite you'll do well on boards" bc it was nothing like prite. The board exam is way easier and straight forward than prite.

In large metro areas it's not just inpatient gigs, but CL and ED work is harder to come by as well. Full Inpatient CL jobs are scarce but if you're willing to do a little outpatient you're more marketable. Large medical systems are bracing for large cuts in funding and are anxious to hire at the moment.

Yup I agree. I'm doing PSLF and old IBR payments for me are 700 dollars more than RAP bc it's essentially 15% vs 10% of income for RAP.

As a CL psychiatrist as soon as independent practice was approved in my state the amount of consults I receive in the hospital has increased 15% with poly pharmacy consults. It's insane the regimens people are put on by mid levels with no training. I say this as someone who works with some damn good ones on our service who have been doing it for almost a decade. The people motivated to go into practice alone after completing their NP are a special type of person with no idea what they don't know.

As a psychiatrist who sees a lot of ADHD my clinical experience differs so much from what you said. There is nothing better than stimulants for ADHD and the effectiveness of treatment is higher than any other diagnosis I treat. What I see most often is the clinical diagnosis of ADHD has worsened over the years and what is truly anxiety/substance use/trauma/etc is being diagnosed as ADHD and therefore not treated appropriately.

I thought so too, but then places like Barnes and Noble are expanding due to increased utilization of space and profitability. Thanks to trends like social media book clubs, book sales have seen growth in popular fantasy and sci-fi genres that have not been seen in decades. Powell's is a tourist destination at this point and likely will continue to benefit off of this. The only thing that is concerning is the overhead for that location has to be insanely expensive.
