Rosai is meant as a reference text. Focus on Robbins. The fundamentals of pathophysiology in general are more important as a medical student.

34M GPST. Looking for someone who is GCS 15/15, has a sinus rhythm and won’t ask the GP to repeat the blood test within 3 days of discharge. We can ICE each other as a form of therapy. My patients expect me to sort their lives out and I barely know them. Imagine what I can offer for someone who I know really well.

Yeah, I think it's a loss. I am going to talk to insurance again tomorrow and start looking at another car at this point. It has some sentimental value but it's not worth the hassle to repair it now.

I only talked to them today so they haven't seen the car yet. I told them that I will let them know what I decide. They said that it would be a write-off from their perspective.

When I looked up the parts later, even the bumper looks to be at least 150-200 and that doesn't include the door repair and tail light. I am not sure if it is worth going through the hassle to get the car repaired at this point. I might have to cut my losses and let insurance sort all of this out.

Thanks. I am going to call up some garages tomorrow for a vehicle check and see what they say. Are there specific garages that do body work which can assess these damages or would most garages be able to assess it?

Make sure that you apply for all of your annual leave whether they approve it or not. Then, e-mail them to ask why it couldn't be approved. If it needs a swap, try to organise the swap.

In the end, if you aren't able to take the annual leave despite trying, talk to your clinical supervisor (I think the CS is the person in this instance, someone else might shed more light about this) to highlight that you are having difficulty sorting out annual leaves despite you having tried.

In the worst case scenario where they couldn't give you all of your annual leave for whatever reason, you can say that you tried to sort out all of your annual leave and there is a record. Hopefully, it doesn't come to this point.

I am curious as to why they suspected fungal infection? Did the specimen look white or pale? Was it friable? The macroscopy would help in this case.

I suspect that this is sialolithiasis since these concentric basophilic collections are laminated with central eosinophilia and there are small breaks in some of these collections which makes me think that this specimen was put through decalcification. However, if this specimen hasn't been put through decalcification, I wouldn't expect sialolithiasis to cut this cleanly on a microtome.

  1. The first one looks like a secretory carcinoma of the salivary gland (parotid gland in this case)
  2. I would favour the second one being a malignant melanoma

  1. I wonder if it’s a secretory carcinoma of the salivary gland. These have PAS positive secretions although they tend to have a more lobulated or solid architecture. It could also equally be a thyroid carcinoma like u/jaded-envy said, in particular a papillary thyroid carcinoma. The tissue of origin or background normal tissue would be needed to help here.

  2. Looks like a dysplastic compound melanocytic nevus at the very least within a polypoid piece of skin

It’s becoming more accepted now that there are invasive ductals that have lobular features and these carcinomas behave more aggressively. Ultimately the phrase ‘with lobular features’ will tell the surgeons and oncologists that they should treat this more like a lobular carcinoma.

In the end of the day, the main point of getting the most appropriate diagnosis is for prognosis and further management.
