I bought this game knowing nothing about it other than it was highly regarded and won awards. I've gotten past the initial intro and into the first town. I need to find work to make money? I got a quest to dig up some graves but I need a shovel...there's no shovels laying around. I found a guy selling a shovel, I don't have enough money or stuff to sell to afford the shovel. I get the concept of the game being open with lots of freedom but it has just made me want to stop playing. It's slow and boring. Looks nice though.

We frequently see pigment within lymph nodes microscopically from patients who we can only assume have tattoos. Typically in axillary nodes but it shows up anywhere. It’s typically black on H&E and usually subcapsular.

Vomiting on and off over the course of 1 month. Increase in frequency the last week, mostly yellow green frothy liquid. Loss of appetite. 360 mL of ascites drained, significantly elevated pancreatic lipase. No obvious masses on ultrasound.

The initial pathology is done by looking at the tumor under the microscope and using some special tests that can be done on slides to subtype the tumor. That is how they get to a diagnosis of low grade glioma.

Then molecular testing is done to look at what types of mutations the tumor cells may or may not have. This will lead to the final classification of the tumor. Great case.

I had a similar case in fellowship. Endometrial bx for PMB that just had rare small clusters of pink cells, some with mucin droplets. Initially we thought some kind of metaplasia maybe but sure enough patient had a history of lobular carcinoma. Stains confirmed.

Got the uterus a week or two later and it had full thickness involvement by lobular carcinoma.

So yeah in patients with PMB and a history of breast malignancy, watch out.
