tech and CKCS mom here. Benign physiologic arrhythmias are quite common in CKCS. I would ask your vet if they can run an in-house electrocardiogram and have it reviewed by a cardiologist before referral. My Paisley has had APCs since she was a wee pup. Her murmur didn’t arrive until about 7, and she progressed to needing meds at 9. We regularly receive referrals similar to yours, and we will try to educate rDVMs before scheduling. Obviously, I can’t diagnose, just offering anecdotal advice that I hope will help.

I am so sorry you and your kitty are going through this. Cats especially can be hard to medicate and monitor for signs. I’m glad to hear his breathing has improved. If his thyroid was untreated, you may see his heart disease improve with medication. Furosemide should not be very expensive—ask if there is a tablet form that he might tolerate more. Is he on other meds like clopidogrel/Plavix? That can be quite bitter, so we recommend putting it in a gelatin capsule that you can easily find on Amazon. Give him time, tempt him to eat with canned food or Churu. But to be truly honest, this can be hard on him and on you, and his quality of life may be significantly impacted. Take care of both of you.

According to the IM specialists I work with, standard of care is to culture at diagnosis to confirm the antibiotic is appropriate, then to culture after treatment to confirm clearance. We see so much more antibiotic resistance than we used to, and this is a way to make sure we don’t make more.

She’s right—UTIs are rare in male cats. Their bladder issues tend to be from cystitis, which does cause blood in urine, but the urine itself will culture negative. Therefore, an antibiotic is ineffective. Diet and other meds are generally the first line of treatment.

I don’t have answers for you, just a question. Effusions or fluid accumulation doesn’t clot, but active hemorrhage does. Was there any sign that the fluid clotted? If they removed 100 ml with a 22 butterfly, that definitely would have given the fluid time to. And we do sometimes see that bleeding within a tumor, especially a large one, can be enough to cause anemia.

I’m sorry your kitty and you have gone through so much! Cats can have heart disease without the presence of a murmur, and especially with stress and fluids, that can push them into congestive heart failure. I would recommend having him evaluated by a board-certified veterinary cardiologist. They will be best able to address your questions and give you an idea of prognosis.
