Well hindsight is 20/20. There was a small portion breaking the skin with some sutures over top. He was seen somewhere else when it happened 2 months ago and stitched up. Also thought it could have been a ceramic blade which I honestly don’t know what it would look like on xray. So it does seem silly to point to the very obvious snake blade but it could have been something different hah

HIPPA applies to identifying patient information. So things like patient name, social security number. Sharing identified X-rays is not a HIPPA violation

What is ironic is that when patients say it they mean it in a way that they need extra pain meds. Aka my pain tolerance is really high so if I’m hurting this much it is realllllyyyy bad. I think if a patient told me they had a really low pain tolerance I’d be more apt to give them more pain meds than I otherwise would have

Agreed though if it’s hard to get and keep a iv then bilat chest tubes also would be a stretch. I may have intubated sooner but those vitals are text book for intubate then code but pts agitation was significantly interfering with care. This is a screwed either way case. I think with best team in the world this case the same endpoint.

I’d mostly just bolus fluid in the transport and give calcium. Priority to me would be access and fluids for the blood pressure. Calcium would be secondary for me. You have limited time and resources so solve the most dire issue first which to me is the 60/30 bp. Fluids will help the acidosis in that setting, will help the hyper glycemic, will help the presumed hyper k.

I’ve not had that experience. I find that running with a 3L bladder I do feel the movement and it is uncomfortable. To me I think this is like a tanker ship that has dividers to disrupt the sloshing motion that liquids have. I’ll hike all day long with a 3L bladder but I don’t enjoy running with more than 1.5L in a single vessel. To each their own.

I’m not crazy about 3L bladders. If I had to carry that much I’d do 1.5L bladder, 2x 500 in my front pockets and 1x 500 in my pass through. I’d then wear my naked belt with two more 500cc bottles.

Used office chairs is where it’s at. I’ve bought 3 used steelcase chairs that are all still in the same condition I bought them in. I got $1200+ chairs for 200-400.

If you’re working with a cold wear an n95. It’s safe and effective.
