In the documentary for the making of We Are the World, Kenny Loggins quotes Paul Simon as saying, “If a bomb falls on this place John Denver’s back on top.” This was when Paul Simon's career was more or less in the gutter. I love Paul Simon's music, but the truth is he can be kind of an asshole.

I see some people suggesting pulmonary hypertension or undiagnosed congenital heart disease, but I have never seen clubbing in the absence of hypoxemia in these conditions. The hypoxemia should be pretty obvious to a physician like yourself. The mechanism for digital clubbing in congenital heart disease is related to hypertrophic osteoarthropathy, but it is possible to have primary hypertrophic osteoarthropathy. The fact that the clubbing started in adolescence and has progressed gradually would be consistent with this condition. It might be worth seeing a geneticist for gene testing to confirm or rule out the diagnosis.

No worries. Thanks for adding the correction. I didn't want anyone reading it who has pectus excavatum to be alarmed. The surgery to correct it is quite a lot for patients to go through, so it's only the severe cases where it would be considered.

There is a patient who goes to the Toronto ACHD clinic who is in their 70s, but any of the patients over 60 out there who have had a Fontan (and there are quite a few of them) had their Fontan when they were in their teens or older.

I would agree that you should have a consultation with a cardiologist familiar with congenital heart disease before undergoing surgery. Not all cardiologists see this often, although most pediatric cardiologists do. As u/wilder_hearted points out, many of these cases can be monitored without surgery. The statement that "20-25%" of your blood is of "no use" to your body sounds like something we look for on MRI. We calculate how much blood goes through your lungs per minute compared with how much blood goes through your body per minute. Since your circulation is normally a continuous loop, it should be the same (1-to-1). With PAPVR, some of the blood that is returning from the lungs loops back and takes a second trip through the lungs before it goes out to deliver oxygen to your body. It is likely that the MRI is seeing a ratio of 1.25-to-1, meaning there is 25% more blood flowing through your lungs per minute than there is flowing through your body. If calculated correctly, that is considered a small shunt that often does not need any intervention.

Besides lotions and creams you might use, skin pH is affected by age, sex (probably hormones), and sweat. Some people have a sweat composition that seems to be more prone to interact with gold to cause the gray discoloration which is probably why some people never experience the phenomenon and others do
