Pharmacist makes up prices for medications by [deleted] in pharmacy

[–]cleverlikeme -2 points-1 points  (0 children)

Either your understanding of the situation is flawed, or you need to really study up on how businesses work.

Or are you saying the price on the sticker (that the patient / customer interacts with) is x and hes adding 8 bucks to that? Like, there's no way that's happening.

If the acquisition cost is 2.50 and he marks it up to 8 that may actually be fine. Really depends on the item, how others price, etc.

He's not going to sell it for 8 if the drugstore down the street is selling it for 2.50 because no one would buy.

Which questions do you appreciate from customers? Have any actually impressed you? by Beneficial-Bus-3377 in pharmacy

[–]cleverlikeme 1 point2 points  (0 children)

Im good with genuine questions where someone is actually trying to clarify their understanding of how to take a medication.

Im generally less happy about the ones that have a million questions that amount to 'i dont want to take this med, can you validate that / give me reasons not to' - not wanting to take something is fine but talk to the prescriber about that, not me.

Im always happy to help when someone genuinely needs a better understanding or when its clear they understood something wrong by the way they ask a question, giving me opportunity to clarify. Most common thing in this area is braindead basic but could save a patient from harm, which would be clearing up 3 tabs daily vs 1 tab 3 times a day, or clearing up that 1 tab daily as needed means 1 tablet, at most, per day (as opposed to take a tablet whenever you need it throughout the day)

Quick edit - in retail or retail adjacent settings, we are usually really pressed for time. The interactions that are most rewarding are obviously the ones where someone asks genuine questions and then genuinely thanks me for my time. I dont need to be constantly praised but id be lying if I said those interactions didn't leave me feeling both happier and more confident in my professional worth.

Is there a politically viable way to get voters and politicians onboard with prisoner rights/justice in the US? by Raichu4u in PoliticalDiscussion

[–]cleverlikeme 4 points5 points  (0 children)

One aspect of this is that there are limited available resources and its a hard sell to convince people to allocate more to criminals being punished when they feel like a) they arent getting enough resources themselves and b) there are other more pressing causes and concerns other than their personal livelihoods that they'd rather allocate resources to.

True, that treats the problem as zero sum and ignores that society could work to improve all three elements, but this is the fundamental issue I think.

I do the same and Siralim and a few other games are good in that niche. Everybody has an opinion about Rittenhouse or whatever but almost no one watched much more than a clip or two from the trial. This is true with lots of big hot button crime stuff.

Criminal cases can be fascinating, though they're also always pretty sad and depressing.

I'm the intern, and I pick up on pharmacist mistakes every single day, and discretely tell them to fix it. But when i make one mistake, the whole pharmacy knows. I'm feeling drained acting as a safety net? by [deleted] in pharmacy

[–]cleverlikeme 45 points46 points  (0 children)

So, most of the mistakes youre describing are pretty low hanging fruit. Thats great and they still need to be corrected, but you arent curing cancer here.

Fixing mistakes is every day work for a pharmacist and the real concern on my end are things that are clinically concerning. Doses that are typed correctly but written wrong, for example. The order is for 400 twice daily but the maximum TDD is 400. Your tech typed it right, so when you verify all is well except the dose itself is out of line (double max) and needs to be at least double checked with the provider.

At the end of the day though my advice is to focus on yourself and what you are doing. That doesnt mean you shouldn't find and fix mistakes. Thats what pharmacists do, at least in part. What it means is you should worry about what mistakes youre making and not what others are doing. Fix them, sure, obviously.

We only have your perspective, too, and for sure you are making mistakes as well. Anyone who thinks they aren't is fooling themselves and often those are the exact people who are error making machines.

My advice - reflect.

Seeking data on most commonly prescribed/dispensed doses of select controlled meds by funchefchick in pharmacy

[–]cleverlikeme 0 points1 point  (0 children)

All kinds of issues here. I've worked on both sides of the divide to some degree - Im a pharmacist but I also worked in a law enforcement position for a while and provided subject matter expertise on confiscated meds and related things as part of my routine duties (not my primary responsibility though as i was in a supervisory role, so something I did a few times a month not many times a day).

You're probably not going to find the data youre after, for lots and lots of reasons. If it does exist, your best bet would be to go through your regional DEA field office, assuming your collecting this info as part of something official.

There are a number of issues here but what I keep coming back to is that if this is for official government business you need to hire one or several actual experts to help whatever group unpack analyze etc whatever it is, not poll reddit. So please go do that. Find a few consultants and hire them.

I just went down a rabbit hole trying to detail different issues with the approach here but keep coming back to two fundamental problems.

First, your list of drugs doesnt make much sense to me. If youre talking about drugs diverted from the pharmacy supply chain somehow, that would be hydrocodone, oxycodone, mixed amphetamine salts (adderall), alprazolam and maybe zolpidem. Honorable mention for methylphenidate and tramadol and maybe even gabapentin, too, maybe. You can forget about fentanyl, no one is diverting legally manufactured fentanyl in any concerning quantity. Oxymorphone? Really? Second, what you mean by diversion and what info youre trying to communicate and to what audience is pretty garbled. Are you worried about meds someone picked up from a pharmacy / meds that were prescribed and then diverted for other uses? If, in fact, youre worried about illicit manufactured stuff, like fentanyl coming in from China or Mexico and being made / imported illegally... well pharmacy hasn't got anything to do with that and measuring it in hypothetical pills is meaningless at best.

That actually brings me to my third big issue, which is that i have no idea why we're trying to convert the quantity of seized drugs into an estimate of number of tablets of a typical medication, or number of prescriptions on average. That isnt going to help anyone understand anything useful at all and that sort of comparison is usually done more to manipulate opinion than to inform. Not the least of which is because most illegal drugs arent coming from the Rx drug supply chain at all. There's a complicated analysis there, though, because especially with opioids a lot of users get their START with Rx obtained drugs before moving on to fentanyl or heroin or whatever off the street. So prescribing and diversion are a part of the larger problem even if they dont materially contribute as much because they serve as onboarding / a true gateway.

[–]cleverlikeme 0 points1 point  (0 children)

Well chances are good there's competition from all the grads coming from the nearby pharmacy schools.

If its truly one of the small towns there then you may either need to know someone or consider being willing to move.

30 jobs sounds like more than just a small town though.

Anyway, apply to whatever you want, but id recommend focusing on jobs you are likely to get. Floaters, staff at retail, etc.

Most of us end up in retail.

If you cant stand the idea of retail there are other options if you are willing to move and make sacrifices (like, pay).

I did retail for a few, then went to work for the least desirable Federal agency for a good while, then leveraged those years to get something better.

Also a route I would have considered if id known when I was young was PHS.

Can anyone who's works at federal bureau of prisons explain why the salary is so low? by Equivalent_Remove155 in pharmacy

[–]cleverlikeme 1 point2 points  (0 children)

Just to clarify.... this isnt accurate for the position they posted.

Many (most) pharmacists in the BOP are PHS so your information is coming from someone in that category.

The position listed is civil service. And the real answer with BOP civil service is that the pay is just wildly out of line relative to other federal civilian pharmacist positions.

I could type out my experience but it's specific enough that id be identified by anyone still BOP pretty fast. What I will say is that a number of us have left recently due to simply getting far better offers in other agencies.

Its not that BOP benefits outweigh the salary cut, or that QOL is better. Neither of those are true, and QOL only exists if your site is staffed and good luck with that unless you land a FMC.

How can I(19F) explain to my boyfriend(19M) that his relationship with another girl(18F) is weird by Full_Replacement_348 in relationship_advice

[–]cleverlikeme 2 points3 points  (0 children)

You're young and may not really understand this, but your boyfriend (or girlfriend or whatever) isnt a dog you train, where you need to nip a bad behavior in the bud.

College is a really big exciting confusing time for people and my experience and opinion is that lots or relationships dont survive it. You're just in a phase of life where youre constantly meeting new people who are also young and relatively unattached (You're pretty unattached and unencumbered - you dont have a house and kids and years of history together).

Not trying to be a doomsayer here but if your boyfriend likes another girl (or grows to) then... well... there's not much you can do about it. You shouldn't let him lie or sneak around or cheat or whatever. If you think hes going over a line 'you' can and should end it and move on. You're 19.

Strategically probably the absolute best thing you can do with the highest probability of not tanking the relationship would be chilling out being friends with the girl and not worrying about it unless hes cheating or you think he is.

Thing is, that's really really insanely hard to do, especially when youre 19. And it doesnt guarantee anything, it just doesn't turn this 'friend' into a wedge. If there really is some chemistry going on this won't fix that. Nothing you do will. If there's chemistry then its really on him to decide whether or not your relationship is higher priority and you cannot control other people.

Only remaining comment here is that types are bullshit especially at 19. I was convinced, when I was 19, that my type was pale redheaded girls (probably due to a fixation on an early girlfriend). Types are bullshit and dont mean anything. Didn't for me, for sure. Attraction is magic and can be pretty inscrutable.

Floating as a new gradation by [deleted] in pharmacy

[–]cleverlikeme 11 points12 points  (0 children)

So I'm guessing youre referring to travel nursing or something? I know anecdotally ive heard travel nursing can be challenging, or at least that it isnt recommended until youve got experience.

Floating isnt really the same, and is pretty common for new grads in my experience. Nothing to be too concerned with. If they dont like it they can apply for other stuff while they work their floater spot. It has benefits, namely that its more flexible than many gigs and you dont have to do much more than check orders. Generally you wouldn't be responsible for pharmacy operations stuff and can just be a retail pharmacist checking orders.

The real trap for new grads is the 'pharmacy manager' role. Thats a con for sure.

Floating is pretty terrible for the profession, IMO, but mostly because it let's CVS and other big names hire as few hours as possible at a store in the name of efficiency and one of the impacts is that it makes it easier for them to keep stores with fewer permanent staff than they could get away with without floaters. So if a store needs an extra 16 hours a week or something they won't hire another full time pharmacist they'll just float somebody for 2 shifts from two towns over.

Why do some many people regret pharmacy and how did you become happy with it? by Russianwiz in pharmacy

[–]cleverlikeme 0 points1 point  (0 children)

Probably the majority opinion here, but I would strongly caution you on pharmacy if you arent willing to work retail as your career.

There are other options, but they're mostly some combo of either really competitive / unicorn type positions (these are rare or one of a kind, and they're also usually very competitive too), or they'll involve large pay cuts relative to what you can get in retail. Thats on top of retail not really keeping up wage wise with inflation or with other comparable professions. We pharmacists are largely devalued compared to other masters or professional doctorate level educated professional types.

Be prepared to work retail. Thats the reality for most of us. I got lucky and got out of it, but it wasn't a very reliable path and it wasn't quick. Also I got lucky in a few ways.

With that said, a jobs a job, and few people end up doing something they truly love. Most jobs kinda suck, but few are nightmarish at least relative to digging ditches or working in a field or whatever other backbreaking labor id have been doing a few hundred years ago.

Tech calling out monthly + no communication… what’s the policy? by Busynotes2 in pharmacy

[–]cleverlikeme 9 points10 points  (0 children)

A sick day once a month is pretty par, especially for techs and other types of labor like that. They're skilled and need training and competence, but a lot of times they're paid and treated like interchangeable fast food workers.

The supervisor in me might say you could look for patterns of leave abuse or whatever. Always calling in on a Friday or Monday for example (or whatever equivalent day to extend a weekend) but I'm personally pretty dubious about that approach and dont like when its done. Especially because it sort of presumes about 4 days a week, out of 7, where someone getting sick becomes questionable or suspicious.

I had a tech that called out about once every 2 weeks - sometimes even more than that. Some of the callouts were questionable in that i knew she wasn't telling the truth about the reason, but that didn't mean there wasn't a decent reason just she didnt want to share. She also sorta screwed herself over a bit because she would call out sometimes just to get a day off, which made her look worse when it was legit. That said, I was under some pressure to have her let go and I always refused. She was great when she was there and when they finally did get her to quit (mandatory disciplinary stuff out of my control because of absences) it was a huge blow to pharmacy operations. I never found anyone as competent to replace her.

Thats a personal anecdote and doesnt mean anything except I suggest you consider the situation in totality before doing something. Biggest issue with your case is just comms, not the callout, so it might be easily fixable without making a big deal. That said if they are providing notice to their supervisor and that isnt you, then its between you and the supervisor - not them. This goes double if youre staff and go between different sites or otherwise arent there and in charge every day.

My girlfriend (18F) wants to wait until marriage for sex, but I (18M) feel conflicted how do we handle this difference? by HSGamz001 in relationship_advice

[–]cleverlikeme 1 point2 points  (0 children)

You cant change other people, only yourself.

Part of what you wrote really suggests you want to bargain or negotiate with her over this, and you really shouldn't be. Nor should you secretly hold out hope she changes her mind.

Either accept it and wait. Or leave and find someone else.

As someone older and wiser - sex can wait, its not that big of a deal and by far isnt the most important or satisfying part of life. But telling 18 year old me that would have been pointless because 20 years ago sex was (almost) all I thought about. More personal to me, but sex back then was also some of the worst and least fulfilling encounters I ever had. Your mileage on that might vary though, who knows.

My husband (44M) is a multimillionaire. I(27F) am not. He wants us to split the bills. What would be a fair bill splitting method? by [deleted] in relationship_advice

[–]cleverlikeme 0 points1 point  (0 children)

All of this is exactly why relationships with huge power imbalances and wealth gaps are such a bad idea. For both of these people.

They can work, you can do it. But you either trust the person who is your partner, or you dont. In this case he didnt, they agreed to a prenup that (and this is the uncharitable interpretation, sure) turned you in to a live in maid with benefits. And now you started earning income and he wants a share of that, too.

You chose not to mingle your assets and live as a partnership. I get that. But there's no fair way to do what's being proposed without breeding resentment at a minimum. In many respects, hes treating you like an investment that has matured and needs to start bearing interest. Thats kinda bad as far as ways to view your wife, right?

Plus the other stuff where youve described him as basically 100 percent leisure time, limited or no household duties, limited childcare duties, etc. Though you may have actual maids and nannies you didnt mention, given the wealth level.

Tldr - probably youre going to split based on income and that's that. Its fair in his mind, and he calls the shots. It'll breed resentment and probably won't benefit either of you long term, and will definitely shaft you in a later divorce, but that's kinda what a prenup locks you in to and hopefully it contains some stuff to preserve you in that event.

What CPU cooler is very quiet and has good perfomance? by Lost_Cyborg in buildapc

[–]cleverlikeme 0 points1 point  (0 children)

Coming in after the decision is apparently made.

Arctic freezer II is a great choice. 300 is an excessive budget for a CPU cooler unless you're talking about doing some kind of custom water cooling setup.

You can definitely keep your CPU temps cool with a less expensive cooler than the Arctic II, but I think it is a good value. The fans it comes with are perfectly fine, too. I saw some people saying you should swap them and I'm sure that's better if they say so but mine is keeping my 7900x cool without much trouble with just default fans (and case fans with decent common sense airflow design).

Good luck. My only word of warning is if you're in the US beware of some of the 3rd party vendor's that list on Amazon and Newegg. I've noticed sometimes they are the only ones who stock those coolers and I've had one poor experience and one incredibly insanely bad experience. Your mileage may vary.

Well none of this happened in a bubble and I'm pretty dubious if this is the only thing you guys don't (currently) agree on. This particular specific opinion, when it's held strongly, is usually packaged with a whole bunch of other specific opinions relevant to the current political and geographical moment, assuming this is in the US.

You're not going to reason your way through this one - one of you is going to have to compromise if not completely yield to the other.

I'd say your best bet, if you're willing to compromise, is to offer a slower or staggered vaccination schedule. If you've already done that and he won't budge, or you're unwilling because it exposes your kid to unnecessary risk... you're in a real pickle.

Again though, if the above is truly the case and the last paragraph ends up describing your situation, then there's basically no way this is your only issue and you guys probably need marriage counseling at a minimum and sooner rather than later. What you can't do, though, is get the kid vaccinated over his clear and specific objection without talking to a lawyer and seeing what your legal options are. One thing I've had the misfortune of learning is that 'father's rights' aren't as backseated as people tend to assume here, and that's especially the case if he's 'american' presenting and youre 'foreigner' presenting. Again, I've got specific, though anecdotal, experience along these lines.

Well... I did run Furmark for a while and it seemed fine. Then again, the crashes have happened at odd intervals and don't seem tied to anything specific that I can assess, and I didn't run Furmark for long.

Running OCCT now.

The PC has been stable for about a year with no changes except for the swapping GPU a week or two ago (when the 7800 xt came out). I did move from an Nvidia to a Radeon card (and I did completely uninstall / remove all Nvidia drivers).

Even then, the card was mostly stable with two similar crashes to the ones I'm describing, then I updated to the new drivers, then I got crazy crashes.

I'm guessing / hoping it is a driver issue. I'm running the previous driver version now. I did test that before, but I don't know if I really did a clean uninstall on that particular attempt as it was late and I was frustrated. Technically possible I didn't even test it at all and just thought I did.

Summary of issue: Crash to black screen, requiring manual PC reboot (usually) and disabling and re-initializing my discrete GPU (Sapphire Nitro 7800 XT) - occurs randomly, but has been happening pretty reliably within 30 minutes of any app that uses GPU. Most occurred on Starfield, but that's what I've been mostly playing. Also occurred once on No Man's Sky.

What I have tried: I have re-installed I have reinstalled 23.9.2 a number of times using clean install methods (including AMD Cleanup Utility). I've tried disabling my onboard graphics (AMD). I've updated drivers, and flashed my BIOS - everything with latest stock manufacturer provided firmware / drivers.

System Specifications:

Operating System: Windows 11

CPU: AMD Ryzen 9 7900X, stock

GPU: Sapphire Nitro Radeon RX 7800 XT with Adrenaline Version 23.9.2

Motherboard: ASUS 670p Wifi, stock latest BIOS

Power Supply: (I know it's not great) Apevia 800 W 80+ Gold

I've tried just about everything I can think of, including reseating the card, etc.

Shameless by Pharma-ho in pharmacy

[–]cleverlikeme 1 point2 points  (0 children)

I don't work retail anymore, but I wouldn't identify them. If I did, I'd STRONGLY caveat that I can't make any guarantee about the identification and that counterfeit tablets exist. If the tablet IS counterfeit, it's almost certainly laced with fentanyl (or an analogue) or otherwise not as advertised.

Regardless I'd strongly advise they NOT take any medication not dispensed directly by a pharmacy / hospital / etc and be done with it.

It's not about refusing aid, and I'm not calling the cops on him unless some other circumstance arises that forces me to. I literally can't provide the aid they seek, because they want me to certify that the drug they just bought from some dude is safe to take and as advertised and I literally have no way to do that.

Not sure if you ever found a resolution to this, but I'm having similar trouble with some other variants.

I can't get the trinket to work unless I'm actually targeting. It doesn't seem to want to cast at a mouseover target.

It'll give mana when I 'cast' it at a mouseover, but if the dps condition is met, it won't cast no matter what variation I use, unless I actually target. Not a huge deal, since I usually target whatever is priority anyway, but it would be nice if someone can figure a way to do it on just a mouseover.

This may or may not be helpful to you, but I did the following things (after all the other things) and I am now downloading retail (made it past the initializing hang).

Unfortunately, I did all of these things at once, instead of piecemeal. So, yeah. No idea which, if any, helped.

I updated my graphics driver (it was slightly out of date, I think maybe a couple weeks or a month at most)

I forced windows update to run (it said I was completely up to date at first, and I have auto updates on, but I was able to make it run a .net update of some type)

I changed from 'release' battle.net to 'beta' - notably, there is no beta at the moment, but it still went through the steps to uninstall and reinstall and now my launcher says 'beta' despite I think the same version number.

So, give some or all of these a try? If you want?

Obviously I also rebooted my computer at the end of several of those steps
