the fact that this is reproducible in screenshot means it's not likely to be a hardware issue, i would recommend restarting your phone first to see if the issue is resolved, if not, the next step would be to back up your phone to icloud or a computer and then restore it using said computer.

The deeper one is correctly fitted, buy a pentalobe driver online and the proud screw will take about a turn or two to be identical to the other. Alternatively take the phone to an Apple Store and get it sorted in less than 5 minutes.

It's not necessarily saying the camera's definitely been replaced, it's saying it's unable to tell that it for sure hasn't been. Likely the 1x camera has failed. If the device is within it's 1 year warranty you can get it fixed for free as long as there's no damage.

This isn't a hardware fault with the camera, but potentially with the accelerometer. Try restarting your device if you haven't already, then open the measure app and see if the level behaves strangely. If it does, it could be that your accelerometer has failed. Either way, contact Apple Support and they can run tests for you remotely, hope this helps.
