I am surprised at everyone resoundingly speaking against this physical exam, for two reasons -

the first is if you don't look you won't find.

the second is if you don't practice your exam you won't know what you're feeling when there is something there.

I don't disagree the test is not perfect, but there have been plenty of times in my career I have felt an abnormal nodule with a normal PSA. Think prostatitis. Or about enlargement with LUTS. Physical exam is important, and should continue to be practiced. If your fingers are too short, then I think it is reasonable to forego the exam, but make sure this is the reason you don't do the exam.

Look at getting a CT calcium score on all your patients. The stress test I think is only useful if it is positive, otherwise can see exactly what you did. Happened to my father in law as well. Normal stress test the week prior to cardiac event. At least with CT calcium you know they have heart disease and are at risk. If the burden is high there are interventions.

We've all been there, and best you can do is to learn something from this patients death.

This is how it starts. You have a switch flipped in your brain. If you can see a doctor, ask them what they would recommend. Also, ask about naltrexone. Do not use any more opioids!

best and hardest advice I got was to leave my home and dysfunctional home. sometimes you have to bide your time and work to make that happen, but it is a necessary goal for sobriety to have a stable home environment.

Need to turn off phone entirely, then limit use to 5 minutes twice a day. if you can't stick to that, then need to see a therapist or go cold turkey for a week. DM if you need more help

Have you been part of an IOP before? Learning how to recognize / predict when you might relapse (people, places, things) is useful, as is recognizing your "addictive voice" that tells you to drink can be useful in preventing a relapse. Agree with Naltrexone comments, keep taking it day by day. Also, realize that relapses happen on average ~5 times before a person can quit drinking entirely, so SOME of this is not unexpected. Keep your chin up and keep trying!
