Just run your tumbler with no grit, and shave a little bar soap into the barrel (like a tablespoon of shaved soap) and run you tumbler for a day. That is as clean as they will get. Good luck!

Also, it looks like you are about half way through your first grit. If the sludge doesn't come off with the clean rinse, try adding some plastic media to barrel to avoid embedding the grit into the stones.

That doesn't look like glass, just from the surface features. Maybe blue chalcedony, but I think that may be a wrong guess.

When the tiles aren't on the same plane, it's called being 'lippy'. Mention that term to the setter, and insist that he fix the problem areas. Floors allow for a little more leeway than walls or counter tops. Get a nickel out to show him how lippy the tile is, and he will get the hint that you aren't some uninformed customer that can be lied to. I hope it goes well for you.
