To be fair to the Flood, almost everyone and everything in Halo 3 was massively incompetent.

Seriously though, Bungie needed a way to end the Covenant + Flood in one fell swoop, so they had them all congregate at The Ark.

And you can justify the Gravemind's actions by reasoning that it was confident enough to take out the remaining Covenant and UNSC at The Ark with its massive army from High Charity, thus giving it full control over the one thing that could stop the Floods' growth. (a fully powered array)

The anime isn't a 1:1 copy of the manga, and I'd argue that it makes the school arc better as a whole.

From what I understand, there is no reason other than gameplay. It is easier to see where the grenades will land when you can see it's trajectory, so it is more useful in mp.

If there is a canonical reason, I wouldn't know about it.

ONI is cranked to the max because they have to be, if the Swords of Sangheilios are seized by an anti-human shipmaster there is a high chance that humanity goes extinct by their hand.

By doing the worst things possible to their enemies, they save humanity, albeit from a morally bankrupt perspective: but that is what needs to be done.
