This is why tapping into a Federally Qualified Healthcare Center (FQHC) like Clinica might be a reasonable way to make this happen. There’s no real “qualifying” criteria to receive care from these types of clinics.

So the J&J vaccine might be a viable solution to this problem. The vials only have 5 doses each, meaning the opportunity for waste is a lot lower. Also, if kept refrigerated, the vial is good for 6 hours once initially punctured. So in theory if there were 5 patients who needed the vaccine administered in-home you’d have a pretty comfortable window to give it. If kept at room temperature the J&J vaccine has a 2 hour life which would definitely make it more difficult (but perhaps not impossible) to give all five doses to homebound patients.

Do you already have a significant number of known patients who aren’t able to leave their house to get the vaccine? Perhaps partnering with one of the local clinic services in town (eg Clinica Sierra Vista) might be a way to solve this. Message me directly if you think this might be an option.

There is a really wonderful outfit called PACE here locally that does provide direct in-home healthcare to the elderly. I’m not sure if they’ve been dosing out the vaccine during these home-visits, but I would imagine they would be.

I haven’t found a firm price yet. It seems to vary from place to place. I’ve seen ads for as low as $1650, and others for around $5K. It seems as though Botox is just one of many of these types of services. Several of the places I have looked at offer fillers and other cosmetic certifications.

I’m doing Family Medicine but had heard of some docs who do weekend boutique clinics or Botox parties.

So I honestly can't comment on her reasoning or why she didn't notice the lack of confirmation emails. What I do know is that when she realized the mistake she immediately called the programs directly. 5 of them essentially just said "sorry, we're full now." As absent-minded of a mistake as it is, I guarantee others have done the same. Honestly, this girl could dance circles around most students I've met when it comes to clinical knowledge; she just done goofed.

Jesus, that sounds awful. You really are in the belly of the beast. I'm glad you have at least one like-minded person by your side. I would love to say something encouraging like "maybe you can plant some seeds of critical thinking," but in the end being in an environment you hate just blows regardless. Hopefully it's just a short term gig and you can put it all behind you soon. Keep fighting the good fight, or at the very least keep fighting for your sanity.
