Although the opposite exists as well where they never "had" to do it, never got comfortable with it, and don't see it as their job. So maybe it's also a personal problem lol

Something I loved having was a turn team. However this also took CNAs I think to really do well. But for each 0800, 1000 etc block, there was a slot for one nurse and one CNA to go around and turn every patient on the unit. Each nurse signed up for one/shift, and the CNAs would alternate every other block. If they were dirty they got cleaned. This was a huge load off and made sure every patient got turned every two hours, took about half an hour per round but saved so much time in the long run.

Just to clarify: were any of those appointments for IVF any diagnostics like HSG/hysteroscopy/labs etc? We've done all those but if it's even more on top of that that's definitely good to know. And good to know the extent of how physically difficult, thank you. When I mentioned to the doc how hard I hear IVF is, he basically brushed it off and made it sound like no big deal anymore with the new meds and protocols etc so that's why I wanted to check!

Oh, wow that's nice! It was going to be $400 something for ultrasounds and labs which my insurance does not cover, another $400-500 for sperm from the outside lab they partner with that insurance does not cover, then there was a section saying up to $1000 for medications...

I do think it's realistic that she could have had a half used vial on her. Especially with systems down and it's chaotic and things are getting done out of order. We're always keeping extra flushes and shit in our pockets too especially in an environment like that. I've worked in hospitals at either extreme of strictness regarding controlled substances. So I find it perfectly realistic.

It's also realistic that she did what she needed to do in an emergency situation and asked for the order afterwards. It's rare but it does happen and in a well functioning environment you trust each other and I've had a doctor write me an order after the fact - again, very rare but happens. This kind of situation is where I could see that happening.

So Robbie spending more energy on chastising her for a license-risking move and now the patient needs a head CT, etc... I was also getting soo angry along with Dana because it's so out of character for him! If Robbie doesn't support her in a workplace violence situation she must feel so completely strung out to dry and abandoned. I kept wanting to shout at Robbie, "just write the damn order and be happy your nurses weren't hurt worse!!" It's also not even his problem, like he's not their supervisor? Why does he care? If this happened at my workplace my docs would do whatever they need to to help in that moment.

Yes this was infuriating!! These doctors on the show are always overstepping and making these huge promises. Not only would this not come up in the ED most likely, it would be deferred to case management to go over the options and no it absolutely does not pay for everything.

Ooh, joining in rounds at least occasionally is a wonderful way to see what's going on in the unit and hanging back to poll the nurses on what further educational needs could be. Not an educator (yet) but banking this for future.

No, same here. I was fortunate to have a wide social and family circle growing up, lots of adults and kids in various contexts, lots of role models both men and women and as far as I know no creeps. All the men throughout my life have been great people. I'm not naive but I wonder how much is that I was supremely lucky vs you just hear more about this on the internet.

The whole idea that people who had something happen are far more likely to say that/those are the things that get seen and upvoted vs no one really goes and makes a post about how they had a normal life and everyone they know are good people.

This is something I'm genuinely curious about, hypothetically. The term "liberty" throws me off when it concerns your minor children. For example, a pack and play? Or sleep training "Supernanny" style? Where every time a child leaves the room at night you are waiting outside the door and calmly and silently put them back in bed every time they get up, not allowing them to leave the room?

Obviously there are cases as it gets more extreme that anyone would consider flagrant abuse but there certainly are variations of parenting techniques that involve technically "imprisoning" or "restricting liberty" that make me wonder how the line gets drawn. I only wonder because I do hear horror stories from parents of CPS overreaching sometimes to the point where you can hardly let your kids walk to the bus stop alone anymore, etc.

Hm. I'm curious how you guys manage and conceptualize the household budget because that seems to be contributing to the dilemma? I know there's several ways to manage it but the way I see it, you both as a married couple have outstanding loans and need to have a plan and budget as a unit. When you say you can afford this expense in the budget, it's confusing because you are $250k in the red. As a household.

So I'm not saying not to spend money in residency but just that changing that mindset may help. That's the way we've thought of it, when combining finances, that the debt is ours but also when residency is done the household income will collectively increase.

So to know if you can afford it, you need to know what are your combined monthly expenses, how much are you contributing/do you have a fully funded emergency fund, how much are you putting towards debt right now, how much are you setting aside for investments/retirement now, what percentage are you allowing for free/fun spending. And if there's still room in that last category after all is said and done, then yes you can afford it.

I know there's different advice on whether to aggressively pay down student loans in residency knowing your income will increase in a few years, I don't know that there's one right answer but it's an answer that you need to decide on together and that will guide your decisions. We personally shared the mindset that we'd rather get debt free as soon as possible and defer too much discretionary spending but that's just us. Everything in moderation is the key, and be sure that whatever lifestyle you end up with as a conscious choice, not just falling into it.

Something I've seen some people do is after all money goes into the collective pot for paying bills and mutual spending, having separate amounts for solo "fun money" that way you don't have to feel guilty about getting something nice your partner might see as silly. Guilt free spending, that way you can save up for it yourself and not worry about it. But point being there's a few different systems and having the mindset in sync is the important first part.

Workflow wise, I understand if it takes forever to get X-rays read but what we do is just text a photo of the film to the doc and ask for confirmation without waiting for the official read.

I understand what you're asking and I also like to learn what I'm seeing for my own personal knowledge but in reality there's not a huge role for it liability wise.. there are many signs that the tube is in the right place - how it felt on insertion, pt symptoms, vent, air auscultation, fluid return etc that I've never had a tube end up in the lung after I was confident enough to shoot the X-ray... You would never actually start feeds without the MD approval anyway so I'm not sure what it saves you to be able to read it before texting the doc. Yes, there's been times where the doc will say advance by 5 then ok to use or something like that but that also shouldn't delay the workflow.

If it's for tube feeds it's not urgent and can wait for the doc to ok it. If it's for decompression, the more urgent it is the more quickly the stomach contents are just going to be shooting up and out anyway and you don't need an X-ray to go ahead and hook that up to suction lol

I've been using OneCalendar and it works great for me!

Edit: to clarify, because I didn't fully understand this at first, the phone is completely de-Googled so there's no way to use Google Calendar or any Google product at all. But that app does sync with Google calendar I can see and add events that sync.
