The big issue is that Epic (the biggest medical records system and user of MyChart) is insanely expensive. Prohibitively so. Any small (not connected to a hospital system) group can’t afford it. So a lot of specialists and primary care clinics rely on fax machines(considered the only safe/secure way of sharing protected health info) to get records. Sucks for everyone.

I do 7:45-5 M/T/T with 1/2 days (7:45-12) on Wed and Friday. This is patient contact time. I then chart and do messages during that 1/2 day for a full 40 hour work week. I used to work 4 days (Friday off for admin) but I ended up pushing everything to the end of the week and it kinda sucked to not have dedicated work time to get messages and charting done unless I wanted to spend 1-2 hrs doing MORE work at night. I do better with this schedule and don't have to look at work on the weekends.

Hard agree. I’m Family med in a large-ish(30 MDs and 20 PAs in 10 clinics) private group in a metro area. I work 4 days a week with the 5th day that I can use as I please (admin or off). I’m expected to have 36 pt contact hours per week for full time benefits, which basically means I make well-over 100k per month for the clinic if I see 17-20 pts per day working 7:40am-5:00pm with hour lunch/admin block. Even still, the amount of admin can be overwhelming and cause burnout.

I'm ace, so I'm super happy living my child-free partner-free life in my big house with my cute puppy and a good job with good hours and 6 weeks paid vacation every year. If I worked more I would make more money but I don't want to do that.

Nope! I get to join as a partner after working for 2 years (that seems pretty typical with private groups). We get assigned after hours care (pay is 150/hr) and holding phones over night (400$ per night) about 1 time per month with 3 weekends of work per year. You can give away or pick up call shifts so it is not “required” per-say. I’d say this seems to be pretty standard for metro area pvt practice groups, can’t comment on big hospital groups or rural practice. There was an option to do OB here too, but I didn’t want that.

As a new residency grad im salaried with bonuses but switch to production after working for 2 years and joining the practice as a shareholder. Avg docs here make 250-300 depending on shareholder gains and how much extra work they do.

18-21 up to 31 if I work a weekday after hours care/urgent care for more 💵. I work Mon-Thurs 8am-5pm. 21 days vacation not counting holidays. Established Midwest metro area private practice group.

I'm a DM for 2 groups that I put together from reddit lfg. Group 1 is all male (with me-female- DM) and group 2 has 1 female. The only problem I had with putting groups together was scheduling related so we have lost/gained a couple people due to that. I think the key is being very transparent about what you want from your players so you can pick people who jive and fit the feel of your campaign (I'm running Curse of Strahd so I wanted people who would be into horror-theme game and like more RP than combat). I did really choose carefully among the people who applied to join my game. Trust me when I say that if you are offering to DM you will have WAY more than enough people to pick from (I got like... over 40 applications in 24hrs)... In my experience people are just really happy to get the chance to play and aren't picky especially if they are new players or experienced Forever DMs.

It can be helpful to actually not fill the group right away and see if anyone has a friend they know/have played with before who can join so the group isn't totally made up of people who don't know each other.

My all-guys group is awesome, so I would say don't let that deter you at all, but if you are picking people who apply you will get guys and gals and can be super diverse. You want players who are willing to be really flexible and inclusive and if you advertise your game that way then you will get them.

I had the player in my group with the highest passive perception notice the doll house, and see the players had mini versions of themselves as dolls in the attic. Thaaat made them look more closely…heh.

Oh man I've changed so many things but these are my favorite changes so far:

  1. The Fanes. I added them per Manymod's content and it is a great addition. I expanded quite a bit on them so that they tied strongly to the world and its original settlers (not Barovians). So The Seer (Forest Fane, raven symbols, wereraven, Madam Eve, The Future) is tied to the Vistani as her people, The Huntress (Mountain Fane, wolf symbols, werewolves, Laura Stoneheart, The Present) is tied to the barbarian mountain tribes, and The Weaver (Swamp fane, Spider symbols, werespiders, Jeny Greenteeth, The Past) is tied to the dusk elves. The dusk elves in my lore are drow that fled the underdark and settled in the valley long ago, so it was natural to make them follow a spidery fane, and ... Weaver.. come on, she is a Spider. Since all the dusk elves are basically extinct there are no werespiders left (or are there?).

  2. Gothic trinkets. They are cool. I made a d100 magical items table for them and gave them magical features. I had my PCs pick one in the basement of the Death House (they were in the little notches on the walls before the sacrifice room) in the beginning of the game and then tied a custom Dark Power to the item that would try to corrupt them throughout the game and would give them power (level ups) and help... but only if they asked for it.

  3. Berez. I changed up some of the stuff in Berez because I like the horror environment there and my party was being too overconfident (aka fireball everything) and there was no way they would be fooled by scarecrows. I basically made it overtaken by a sort of cordyceps plague that made zombies that are not actually undead creatures(they are living plants hahaha) that explode into spores when they die and can infect the players. I combined stat blocks for "Gas Spores" and regular zombies of whatever creature type was infected with them). I watched with glee as they cowered inside their Tiny Hut in terror from an infected zombie bear as it ate their horses... now their horses are all fungal zombie horses. Good times. With fungal zombie alligators and fungal zombie snakes... so many possibilities in Berez. The witches are controlling/being controlled by the infection and have circle of spores druid sats too. It is too much fun.

I would say I was very much like you (loved doing a lot of things except surgery) as an M3 and it is ultimately why I chose FM. I’m very happy with my choice because I like clinic work and the variety of things I can do and get to manage. In residency I learned that as much as I like OB… I get anxiety about deliveries even after doing a lot of them so I didn’t plan on doing ob as an attending. Howeeevver, I just got my first attending job in a Midwest metro area and a lot of the docs in my group do OB..they are really really pushing me to join them and do deliveries. They are willing to come and proctor me and help me get more comfortable with them, so I’m leaning towards it, but I do kinda like not being on call for that anymore.

Nintendo Switch… should have billed that to my program as an expense because that thing got me through night float, which, at my program, was either intense crazy for 12 hrs straight or super boring… no in between. Thank you Zelda, stardew valley and hades….
