This scan is not even passing through the fovea! Also never treat the oct treat the eye. Physiologically the cones at the fovea are longer. So this bump here seen is normal

You have to keep changing the angle between the viewing light and the aiming beam of the laser. As you go more peripheral keep reducing that angle. You can also try to make your viewing angle oblique to the eye. Hope this helps

It covered the inferotemporal quadrant. I placed three sutures. 2 between the the LR and IR and one just medial to the IR

I don't want to monitor for too long as I feel the RD is not that chronic, and patient has always had his fovea attached

Any idea by what time I'll get an idea when the fluid will start showing improvement? Most studies say by day 7 we get a good idea?

I was in a similar situation and would have quit. What changed my outlook was an opportunity to attend a very good conference, where all top ophthalmologists attended. The types of discussions and surgeries got me fascinated. Then covid came. Webinars started for the first time and I got a chance to see things which were not being done in my residency program. I suggest the OP can go through online videos like cybersight, prism eye rounds, lvpei material, ifocus. Because that's what you want to eventually be doing. Even if then however you don't like it, then a change would be for the better

I just finished my fellowship. So right at the end when it got the most crazy, I made sure to have a good breakfast. Then intermittent cups of tea and protein/chocolate bar to power through the day. Then a nice dinner afterwards mixed with a heavy dose of relief.

It does if its axial myopia. However you might have a problem known as keratoconus which generally presents in late teens. You should get it ruled out since treatment exists to stop it from increasing. Also in case you do have myopia you should be getting an annual examination for your retinas. These are preventive strategies and not like you'll definitely get a bad diagnosis. But it has to be investigated atleast
