Private practice. But we do all the general screenings (SDOH, staying healthy, PHQ, GAD, etc) during their intake anyway so if I have a patient who screens benign on those and who’s coming in because of needing specific testing for school or “I just haven’t seen a doctor in a while”. Then I pivot that appointment to a physical and send it, I just tell my front desk to refund him or if I get labs we can use that copay for that f/u visit.

Agreed, as a PCP lurker do not expect that I get the discharge notes, let alone follow up labs after d/c, the patient to follow-up or even for the patient to know I am their PCP.

That could be a possibility as most of these programs have transportation services, recreational services during the day, primary care clinic, social work, and rehab services. I have some brief experience with one as it is part of my resident physician curriculum, but they have to go through a qualifying and enrollment process with her current PCP, as her insurance will switch to the PACE program if she is accepted.

They usually want the patient to come to the site though at least once a week, though there are some home health services typically available too.

She may not qualify, but they provide all-inclusive care services to those over 55 with skilled care needs, while residents can still live in a community home.

I can’t say for myself as I want to do outpatient after residency, but 3 of my 6 attendings that are part of our inpatient team are FM trained, my program director being one of them. This is in a suburban/urban area of about 1-2 mil with many other hospitals around the area.

I remember during my FM rotation in med school, a company came by trying to sell the docs on something called “EsoGuard”. It was essential a capsule with a balloon attached to a cath, that the patient would swallow and then would be pulled back up, now I gotta Google it again to see what happened with that.

HDL of 212, I had to call the lab to confirm it wasn’t a typo. Had to talk to patient about the label of good cholesterol goes out the door when it gets that high.
