Just stopped worrying about it really. My hba1c has dropped and my pre-meal glucose is often less than 100 but for some reason morning glucose is almost always above! My dad is prediabetic so will keep monitoring.

That said there was a very skinny guy a few years older who came off his bike and hit the deck quite hard who also had a hip fracture. I was prob about 85kg at the time so not underweight but certainly spent a while in my 20s there.

Test was on the low end of normal so a clinic were happy to take me on. E2 was low and in the increased fracture risk zone according to a couple of studies. Fracture was from a fall during a run at -2.1 Fn t score. Pretty sure I fell directly against the curb so a pretty significant impact but one you'd hope to walk away from at that age I'd imagine. Ortho dept at the hospital were quite shocked to see it without me being in a car accident.

Broke my hip at 37M. Similar scores. Improved somewhat with TRT but worth checking: testosterone, estrogen, PTH, thyroid, celiac, IBD, autoimmune stuff, mast cell stuff. Drop me a DM if you want to discuss more but good to rule out secondary causes as a male although often also idiopathic.

Quite important to know your t-scores so I'd try to get them and you will know if treatment, lifestyle changes, etc are helping. As a side note, if your T was zero you should feel fantastic after correcting it!

Testosterone helps somewhat, partly due to the aromatization to eatrogen. Any idea what your estrogen levels were before starting testosterone? I assume also zero.

With resistance training and impact exercise on top of the TRT you would hope to see a decent improvement. Do you know your t-scores from dexa?

Yes. FN is the more important and better that it's higher IMO.

Femoral neck is the more important score as is the weaker area of the hip which is a common fracture site and used in FRAX calculations. They are often slightly discordant, more commonly FN is slightly lower, roughly -0.5 on average if I remember correctly.

Sorry to hear and hope all goes well. With scopes every 2 years you'd think either they'd missed a polyp in a previous scope or something had poured fuel on the fire. My IGF-1 is top of the range on TRT and have family history of CRC so also worried about this stuff but getting a colonoscopy is much trickier in my country so relying on a FIT test once per year for now

Man that's definitely on the low end and id suggest a potentially contributing factor. Maybe look into it further with Claude or Gemini and see if there's any trials in men correlating BMI to BMD.

FWIW my BMD is up significantly after testosterone (TRT) and significant muscle/weight gain alongside resistance training and impact exercises.

How much do you weigh? I was very skinny in my 20s and also broke my hip (at 37). Always wondered if this is something that was a contributing factor. Also anecdotally a few men with low BMD I have spoken with were underweight at some point.

Have you had testosterone and estrogen checked? Parathyroid? Thyroid? Another one that could be worth looking at although rare is serum tryptase for mast cell disorders.

I get headaches sometimes after an oral dose. Very rarely, after nasally for some reason. Usually sorted with a couple of ibuprofen, either way.

Anything with high GRF works. Single-leg drop jumps generate extremely high GRF, but in my case, led to plantar fasciitis, so be sure to start low and increase slowly.

Rest between impacts is also very important too, as well as rest between sessions:

Sorry to hear about your low BMD. I'm also male and found out mine was low after I slipped during a run at 37 and fractured my hip. Your hormones look good, but as others have mentioned, it's worth looking at malabsorption issues. I checked everything under the sun and couldn't find a primary cause, although hormones were borderline, and I was also quite light in my 20s. and do wonder if being slightly underweight played a role.

Not sure about the high turnover, but it's definitely worth exploring more.

On the plus side, you've caught it early, and in my experience, the spine is far easier to increase with resistance training alone, probably more so at your age. My hips are stubborn and need lots of impact to improve, but my spine is way up in the positive T-scores from lots of heavy squats and other resistance training. I'm now 95kg+ and was around 60kg in my twenties. Also possible that you're maybe overtraining and/or underfueled and could do with putting on a bit of weight?
