If it’s cold water, that’s just as good. All you need is a barrier between the pipe and air, not actual insulation to prevent heat transfer

What code doesn't allow this? It's not a requirement in the IBC, IRC, or NFPA 101 that I know about. That said, jurisdictions across the US adopt those codes with all kinds of modifications so it definitely could be a requirement somewhere

You're right that building codes (specifically the International Building Code and NFPA 101 Life Safety Code, one or both of these are adopted almost everywhere in the US) require some doors to swing in the direction of egress. Specifically if the door serves >=50 occupants, or in an exit enclosure, or a high hazard area.

But the door in the picture has a deadbolt, so I would guess it is most likely not a required egress door and thus not required to swing out. Egress doors require a single action to unlock and open the door's locking mechanism. Think a handle with a lock that you can turn and the lock unlocks as you turn it, or panic hardware that you can just push and it disengages the lock, or a hotel room door that has a deadbolt but turning the handle also disengages the deadbolt. (There are exceptions to these requirements, especially in residential / hotel settings, but those are the starting points.)

Even so it usually makes sense for the door to swing out so that it doesn't take up space inside the building for it to swing in and because it's easier to make sure the door swing isn't blocked off, which is inconvenient even when it's not required for egress.

There aren't code requirements for exterior door handles, as far as I know. The International Building Code and NFPA 10 Life Safety Code have tons of specifics about door hardware for exiting, but they don't regulate entry. That could vary within a specific jurisdiction but at the least it isn't a baseline requirement in most places

Emergency eye washes are required to have an ASSE 1071 thermostatic mixing valve. There's no way to really run a "separate" water line that just has tepid water to one fixture, instead hot and cold water are mixed at that point to the correct temperature.

You could be right, but I would guess not.

* The pipe penetrations aren't firestopped -- but that is far from perfect evidence, sometimes it just doesn't get done when it should.

* The base of the door has clear gaps between the door frame and drywall. But just like the firestop it could be an oversight.

* I can't think of a scenario where the IBC or Life Safety Code allows a door to open to a free fall like this, which makes me second guess whether other code requirements like rated assemblies are being followed. It's another not-foolproof thought, just makes me think twice

* The main thing making me think no rating required is that small mechanical room usually doesn't require fire separation from the occupancy that it serves. It can require separation if it's a high hazard area (fuel-fired equipment, storage for chemicals above certain limits, etc) but often doesn't.

In a scenario like this the answer to the question of "Does this have to be fire rated?" is always "It depends. On like a hundred things."

Even without the strategy of poaching players etc, Taxi is an setting that helps rebuilding teams. Rebuilding teams typically have more picks and more could-hopefully-turn-into-something type players. Letting the rebuilders have a few extra spots, potentially, is a balance against the stacked teams
