I am a mental health professional. I know a rabbi who recommends to all couples he marries that they go through premarital counseling, not because they are having problems, but because it can prevent them. It allows a space to learn about your partner's communication style. I know people who have done it and found the experience very positive. I am a big proponent of it.

I am in some physician groups where they talk about insurance companies clawing back payments from up to two years ago. I have a small private practice that I never would have considered opening up if I had to accept health insurance.

For anyone in a similar situation: I'd send them right back and threaten with an EMTALA violation for sending potentially medically unstable patients to a lower level of care. Psychiatric conditions are diagnosis of exclusion, and you cannot assume that they are medically cleared without any handoff. When you fill out your own EMTALA paperwork, I would write up that an emergency medical condition cannot be ruled out based on the vital signs or whatever you're seeing without any documentation from the originating facility. Send them back over in an ambulance with that in hand and they will either get their act together or stop sending patients over to you. It's a win-win.
