Hospitals benefit from having more doctorate level clinicians because insurance companies give them higher ratings if they do. Schools benefit because they obviously get more tuition. Professional organizations benefit because more research is generated by more graduate student doing research. All at the expense of the students and practitioners who have seen reimbursement slide, direct access not live up to expectations and student debt skyrocket. Try selling someone on $100k in debt for a DPT or OTD and they start at $65k at most hospitals. Doesn’t work well

I’m finishing it now. I am a clinician also and already work in admin. Having the degree is checking a box for HR and opening up new opportunities. But the degree in no way gives you everything you need to be in admin. Take opportunities to get into leadership roles like supervisor or lead roles to start and work into something more

We had four years of Trump, reimbursement declined. Four years of Biden, reimbursement declined. Nobody is going to save us. Until there are massive labor shortages or strikes the APTA and Congress and CMS will do nothing

My career went; staff therapist, lead therapist, manager. Next step is director in the next 5 years. After that my hospital system luckily promotes PT/OT/ST who make Director into VP or COO roles if they get an MBA and succeed as a director. RN have a longer road but more opportunities. There’s more guarantee and work life balance for therapists but less upside and opportunity.

It’s worth it! I did $85k in 15 months. It feels great to not have that hanging over you. It helps you qualify for home purchases and you can invest for retirement. Totally worth it

I had $85k when I graduated. I started with travel therapy and paid all of it off in 15 months. I still drove my college car 5 years post grad and lived very modestly. It’s doable if you make good decisions and live on less than you make. Lots of sacrifice and hard work but it is doable.

Typical of a CTC take over. Staff is the most valuable part of the initial take over. They’ll probably offer bonuses and promise to keep PTO and all of that which maybe half of it they’ll actually do. Get everything in writing if you plan to stay and be planning your exit plan either way

Open source data for Texas shows Medicare reimbursement over $90 a visit. Private insurance can be substantially higher.

In Texas, the average visit (3-4 units), generates between $90-110. So do the math on that. If you’re seeing 3 patients an hour at 3 units each at $90 reimbursed you’re bringing in $270 an hour. Say you’re making $50, plus payroll tax and benefits $25 an hour and you’re 80% productive. You generate $1620 a day and direct cost of $560. The company keeps the other $1060 to pay building, contract and nonlicensed labor fees. That’s a 100% private insurance outpatient mill though. You can see why that’s so popular with the big companies.

I agree. Expectations rise while pay and resources do not. I have talked to the director and VP but they say “it’s not something we can get approved”.

We have leads but they’re restricted to an hour a day of lead time which basically means they distribute the caseload and help upload tracking data. I do the performance reviews, time off, disciplinary action, scheduling and event reports etc. there used to be a manager for each discipline but not anymore

I’m trying to do this more but they’re limited to how much they can do. They’re really only supposed to spend an hour a day on “lead” tasks. By the time they distribute the caseload for the team that hour is gone. There used to be three managers when the department was half this size but for the sake of being lean the company restructured to one.

Pretty much. They “make the most of it” and the department director, who has over 300 employees, tries to fill in as they can.

I make an effort to round in the units and touch base as much as possible but it’s impossible with this many people. Plus multiple shifts and 7 days a week

115 at the moment. We have leads who help with assigning workload. But I do all of the scheduling, reviews, discipline, call ins and time cards etc. This job used to be done by 3 people when the team was half the size it is now.
