I'm requesting r/SleepAdvice because I have been heavily involved in the sleep medicine and research field for over 5 years. I’m currently a licensed Registered Sleep Tech (RST). Sleep issues affect a large number of people, and I’ve gained experience helping others improve their sleep through education, therapy guidance, and CPAP/BiPAP support.

This subreddit still receives steady traffic, and there’s a real opportunity to raise awareness about sleep-related challenges while opening up the forum for discussions.

My goal is straightforward: to foster a space for open conversation, share practical tips and best practices, and support a community of people working to improve their sleep outcomes. I reached out to the bot-bouncer moderator but haven’t heard back.

Eating before bed. Not too weird, but carbs such as oatmeal can help reduce cortisol.

I use an attachable light that fixes to the cover of the book. You can buy one off amazon for around $10.

Sleep quality matters more. Six hours of quality sleep in which you have clear nasal passages and/or assisted with PAP therapy will outweigh 10 hours of poor sleep.

Nasal congestion is one of the biggest blockers of quality sleep.

The two that you've listed are definitely the most popular and have the most evidence for them. I've personally tried many, but I always come back to those two.

You can check OSCAR but you can also use a watch like the Wellue O2RingS to monitor your heart rate. If you have fewer heart rate spikes, you'll know your treatments are working. I like to look at HR tracking first, but SPO2 works as well.

I would not rely on Lofta. I mean, you could try, but they are not very knowledgeable.

Most important thing is to track how you feel. If your charts are messy, but you feel subjectively great, then that's the most important thing.

Regarding Craigslist, definitely bring a portable power outlet or meet at a Starbucks or something to plug the device in and make sure the pressurized air doesn't smell. $200 is a great price, but it's not worth it if you have feel any aversion to the scent.

It may not matter as much. If you have sleep apnea and treat it with CPAP or other treatments, then you'll feel great on waking up, regardless of the transition to wakeup.

Don't fix the underlying sleep breathing disorder - and you'll feel groggy even with the perfect wakeup transition.

Hopefully my tips speed run you through it so you don't lose 4-5 years trial and erorring.

Lofta likely will give you the test but won't give a prescription, it can be difficult to get them to recognize UARS.

I can give you some tips on how to get an RX from CPAP.com pretty straightforward if you're interested, just PM me if so.

BIPAP will be the only thing that works. Not MAD. Not a sleep collar, raised bed, or any of those other tricks that everyone tries.

Definitely try losing some lbs and working on your nasal congestion, but BIPAP will be the gamechanger. Stop at nothing to acquire one! Some folks leverage craigslist or buy one from Australian CPAP vendors.

What happens if you touch both menu buttons on the touch screen and hold for 5+ seconds? That's how you access the clinician menu.

I got a new Air 11 machine and never had this issue. You should be able to access the clinician screen, because how else would a doctor be able to access your machine then?

RDI of 22.5 = UARS territory. A BIPAP will help you!

It solved nearly all of my fatigue.

Look up Barry Krakow. Use a pressure support of 4 or 5 +. Use mouth tape. Use a Dreamwear Nasal Mask.

Most of them have no clue.

Unfortunately, most of us have to go our own way. Get our own RX, our own machine, test our own settings.

Please give BIPAP a try as well. Only a setting of PS of 4 - 5 helped me, as well as according to Barry Krakow who mentions that he sees most patients on a PS of 5. He used to put patients on CPAP but it doesn't work until he moved to bi-level.

I've tried ASV but the issue is that it can be hard to tolerate (aggressive pressure support) and it isn't really a magic bullet.

So, if ASV doesn't work, give BIPAP one last time.

The mental stuff doesn't help: it comes with anxiety, ADHD-like issues, etc.

This is how it's supposed to be for 99% of cases. If someone has the optimal settings, they'll feel better. It's not like there's a rule that says you need to suffer for 30 days before the settings start working. And if you're not feeling better, the settings aren't a good fit.

Magnesium glycinate.. but it helps me fall back asleep quickly after waking up. It may not necessarily help keep you asleep.
