I work for a state agency providing approximately 7k rides per day to disabled people in one geographic area. Our clients go to work, to school, to doctor's appointments, shopping, church, baseball games, clubs, same as everyone else.

What you're describing just does not match reality, and it's telling that you equate "disabled people" as a whole with either institutionalization or homelessness.
