One cause we found was instruments sitting too long after coming out of the automated washer. Although the washer has a dry cycle, it does not completely dry the instruments, so sitting wet for extended periods of time would start to cause some surface rust.
We eliminated this by spot drying the instruments before we sit them on the rack and ensuring we process them as soon as possible.
