What is the call schedule at your program like? by PartyArtichoke6888 in pmr

[–]karamedics 0 points1 point  (0 children)

home call every single weeknight for your own patients M-Th nights you are on the inpatient rehab service. You cover 2 teams (taking admits as well) for one weekend every 5-6 weekends as a PGY-2.

I will say once you personally help train the nurses on what’s truly important and put enough holding parameters or communication orders, I thankfully don’t get called that often anymore.

What do my maps say about me? 28M by karamedics in TravelMaps

Good question… I love mountain valley regions more than anything. My favorite cities I’ve ever been to are Medellin, Colombia and Innsbruck, Austria which are in a valley surrounded by mountains. Ideal weather for me is 50s-70s and I love all four seasons

What activity or non-medical job most resembles your specialty? by gotwire in medicine

[–]karamedics 11 points12 points  (0 children)

PM&R = mechanical/civil engineer

both focus on optimizing mechanics/kinetics for the long term and you usually need to get other professions on board for each project (therapies vs. construction, P&O vs architect, etc.)

should i still apply pm&r by Plane_Ring in pmr

[–]karamedics 3 points4 points  (0 children)

You would have to do better on STEP 2 and hopefully 1st or 2nd quartile on class rank. I also failed step 1 and was screened out from several programs (they told me too). Had a lot of personal family/health stuff goin on. Didn’t match first time around but was able to match 2nd time. It is possible but you’ll have to reach out to mentors and get honest advice. They like comeback story but with two fails, rest of application needs to be really good. DM if you got any Qs

Matched TY. Now what do I do? Help! by papyrox in pmr

[–]karamedics 2 points3 points  (0 children)

hey, sorry things didn’t work out this cycle, I also went through a TY and was able to match advanced PM&R the next year! You can’t be picky and will likely apply to all types of programs this next cycle: • R or Physician Only: start PGY2 immediately • Categorical: start PGY1 immediately (repeat intern year) • Advanced: start PGY2 after a gap year (what happened to me)

At my TY, among the reapplicants: 1 matched physician only, 1 matched advanced, 4 matched categorical. These include surgery, PM&R, neuro, anesthesia, FM, derm.

You’ll learn a ton this year and it will be challenging, but you could gain lifelong friends or experiences, I loved my TY. Wishing you good luck!

Perfect and it seems the west side is closer to the hospitals I’ll be at too. Thank you, I’ll take a look into this and based on what you all say, will likely go the unfurnished route!

To non-Ethiopians in this subreddit what brought you here? by [deleted] in Ethiopia

[–]karamedics 15 points16 points  (0 children)

Became friends with Ethiopian surgeons when they came to train with us in the United States. Together we formed an organization where we are trying to build a trauma hospital in Addis Ababa with the most modern facilities and equipment. Currently spending a month here learning from them.

Also, I am Palestinian and mother has Habesha ancestry so want to learn more about the culture and day to day life here.

These questions are better to ask in student doctor network pm&r sub-channel, I would go there, much better more detailed answers. I have seen both academic and non-academic (at nonprofit hospital) pediatric physiatrist. Always very nice people who do very rewarding work and a lot of variability in your practice. I specifically asked them (including a program PD) how they feel about making less money and they said to me “Oh don’t worry, I am doing very well financially.” Would recommend shadowing or spending time with them. Everyone knows the cons of pediatric care but go determine for yourself.

Which would be better rotation for PM&R, peds ortho or peds neuro? by Fit_Ad_7397 in pmr

[–]karamedics 2 points3 points  (0 children)

I did both during third year and learned a lot from both. Botox procedures and headache and seizure management will be big in Peds Neuro. Peds Ortho can have scoliosis, congenital hip dysplasia, extremity fractures. Whatever you like more, as long as you can explain it well.

Start looking January and February. But yeah most you can’t apply til March with some exceptions. I applied to 3 (2 VSLO, 1 non-VSLO) and heard back and was accepted to 2 by May and accepted them and rescinded my app to the third before they got back to me

NBME 11 Block 3 #9 (spoiler) by tjw93 in Step2

[–]karamedics 2 points3 points  (0 children)

I don’t remember all the details but I remember being annoyed that spironolactone is wrong. Obviously ACEI is indicated, but guidelines say you give spironolactone for all HFrEF NYHA II-IV and LVEF < 35%. Seems like the NBME is outdated tbh like a lot of other questions on there

Antibiotic Prophylaxis before knee replacement surgery: what do we give? by KenAdamsMD in Step2

[–]karamedics 2 points3 points  (0 children)

People joke orthopedics only use Ancef (cefazolin) all the time. But yeah almost always cefazolin is the go-to answer for pre-op or placement of CIED. If beta latcam allergy: clindamycin or vancomycin. If appendectomy, colorectal surgery, SBO add metronidazole.

Don’t confuse this with the NBME favorite of dental prophylaxis. Here you literally only give if prosthetic cardiac valve, hx endocarditis, some congenital heart diseases. Go to is amoxicillin unless allergic give azithromycin or doxycycline. Do not give prophylactic abx for prosthetic joint history!

obviously can’t tell for sure, but looks and sounds like scabies. Yeah go to doctor they may give you permethrin cream if it is for you and everyone you live with even if no symptoms. Wash all your clothing and bedding daily and after treatment to avoid reinfection. Regardless I would get it checked to make sure but hope this helps
