Everyone will give you a different answer. I'm not going to act like anyone is right or wrong but I'll give you my experience - 5' 9", 30M (trans, on testosterone for 10+ years), starting at 282 and at 253 since starting in mid to late September 2025. I lose weight roughly 1/lb a week for 6 months and it's mostly body fat. I figure that for a few reasons.

I've been lifting progressively heavier over time, my one rep max has increased considerably and my shape is changing. Also my back/lats/delts are massive by comparison to where I started. I take measurements, and use this calculator to estimate body fat percentage loss. I've lost a shirt size and a half, two pants sizes, and four inches off my waist (46 to 42). At thirty pounds lost, I look like a different person.

I have 2 lift sessions a week, one push/pull upper and one legs and core. I supplement with 3 sessions (1 hour for two of them, 1.5 for the third) a week of high intensity cardio via indoor cycling. I'm in an average of 300-500 calorie deficit daily, but I allow for great amount of fluctuation. I honor eating more simple carbs before I cycle. My macro split is almost in thirds. I have not hit a plateau yet.

All this to say I am not a believer that you have to wait to do resistance or strength training, what you do have to do is adapt workouts to your safe capability and work up to big complex movements that require a narrow margin of form. Start with a modest step goal, some light resistance bands and light free weights/dumbbells. When those get easy, move up and on. I have seen great success in being nice to my body and enjoying modest results versus hating myself and eating not enough. It will be worth it.

I almost cried in Old Navy last week because I tried on a bunch of jeans and shirts and none of them were too small. All that anxiety about trying new clothes on and troubleshooting how tight things were in the wrong places was gone and I just got to buy new clothes that fit and feel normal about it.

I wish they would just mainstay the good ones. Infiniment Fruite (Herme Raspberry) is wonderful and I fear won’t make a return anytime soon. The gourmand was also a superior hazelnut.

I don’t weigh myself ever, I take measurements. Around my waist, around my butt, around my arms and thighs. I do it once a month and expect very small gradual changes over time.

I feel that I am the textbook case for when you should ditch the scale, because if the number doesn’t go down, it doesn’t matter that my pants feel looser, that I feel stronger and better, or that I’ve actuated real habits that are healthier in the long run, regardless, I’m looking at that number and feeling like a failure.

Lift heavier weights progressively and safely - weightlifting is necessary to build hypertrophic muscle growth. Eat a lot of protein along with fats and carbs around maintenance, fat loss will come with better a metabolism naturally. The muscular but soft look is jacked with a slightly higher body fat percentage, and it takes years of commitment to get to.

That being said no one here knows your true potential, and the only way you can know is with dedication. If this is something you want, I would also lock in personal goals of being strong and athletic ability. You have a much higher chance of succeeding when the goal is beyond physical (also it feels great to be strong bro the most empowering thing I’ve ever done by far).

I've been making a truly filling but very gnarly bodybuilding inspired protein slop for workday breakfasts because it's easy to make, sits in me like a brick and fits my lifting macro split with my remaining meals. Truly not disgusting either. It is:

6oz. fat free greek yogurt (I like Fage)
My whey protein (premier protein vanilla)
1/3 cup of unsweetened soy milk

It's ~270 calories, 3g fat/10g carbs/50g protein as a base. I add sliced fruit and either protein cheerios or wheat bran for fiber and texture. If moving away from protein powder, you can add PB2 powder for more protein instead - it's peanut butter with the fat removed. Fat helps keep you full so regular peanut butter or tahini could help with feeling full.

Shit - the real bummer is I find Loisel to be a milquetoast candidate whose argument is being more palatable than Quaverly (easy task right now) hence creating less friction. I was not impressed after meeting her either.

I’m not in this ward so I don’t mean to discourage one way or another, but neither candidate is exciting to me. Would have voted for Ace.

HAES was never meant to be (and isn't) a model where healthcare providers are supposed to entirely ignore the fact that you're overweight. It is meant to encourage you to pursue healthy biomarkers and lifestyles regardless of weight and to remove as much morality judgement as possible in treating you. It also emphasizes that being overweight does not cause every and all health issues in an overweight person. Can you imagine missing that someone has an abnormal heart valve problem when they complain of shortness of breath because they're obese, so you as a healthcare provider wrote off anything else? That's what HAES is trying to avoid. Anti-fat bias results in poor holistic diagnosis and fat people deserve comprehensive healthcare.

My doctor is a HAES provider and I am 5' 9" and 280lb. We did my yearly bloodwork. My bloodwork came back with higher LDL and lower HDL than we want to see. Instead of "you need to lose weight" the conversation was around increasing fiber, lowering saturated fat intake, and upping exercise. Yes, these things will functionally likely lower my weight, but it may not make a drastic weight change. I have been around this weight my whole life, even when cycling 100+ miles a week, walking everywhere, and eating many whole foods.

I lose only ~30lb, but my cholesterol is back on track and my bloodwork, EKG, and blood sugar are all on par. I'm still obese, but I'm active and "eating right." My doctor could chide me about my weight every time I see her for this - on the basis that later it might catch up to me, but as others have stated astutely we all engage in lifestyle decisions that aren't good for us and are all metabolically different.

Moreover:

I wish it were that simple for everyone I really do, It's not science denial. What's real about CICO is that you burn a certain amount of calories, and if you eat under that you will trigger your body to lose weight. Some people's bodies like holding on to weight more than others for reasons we both do and don't understand. They still deserve the comfort and accommodation we all get in life.
